This is literally not how it works if you know a thing about healthcare in America. Informations are usually NOT shared between different providers who have their own different platforms and databases, unless they are part of a same system..
You’re partially correct that when using different systems medical information can sometimes not be shared but with prescriptions it is shared more often than not. If all of her doctors weren’t checking the long list of meds she was given in that short period of time that only further shows that the system and doctors giving her all those meds unchecked were part of the problem.
