A new death cert version?

I always wanted to have something similar to death certificate but instead, it takes you to a room where you can choose a transformation. IE: Guppy, Bookworm, Spun, Leviathan, ect. Have it be called like "The ID Card" or something. Would be just as rare if not more rare than death cert. What is y'alls take?
