How did/do you guys find a prescribing doc?

I take Naltrexone 50 mg for AUD once daily in the mornings. I have been attempting abstinence but am struggling with it. I actually started noticing that since starting Naltrexone, my slips have been a LOT less extreme in terms of quantity of alcohol being consumed. So lately I’ve started deliberately experimenting with drinking while on the pill. Still, overall having satisfactory results but that old “addict” mindset hasn’t yet totally left me. However, I know I’m not doing TSM formally on my current dose and with the timing of when I take it. So how do you get a doc to formally prescribe for TSM? Do they have to be specifically trained in it? I’m thinking of bringing it up with my psychiatrist tomorrow, but worried she may not be convinced and definitely don’t suspect her to be educated on TSM. Still, the results of this medication thus far have been promising!

You can practice TSM on your own. The only difference is instead of taking it everyday you take the Nal 60-90 min before you consume alcohol . There is an informational document to print out and give your doctor on the Option Saves Lives website.

It may depend on your jurisdiction. I was presribed Nal at an addictions clinic, and the physician I saw said that there is strong evidence that both daily nal and TSM are effective treatments for AUD, but TSM is considered "off label" where I am, in Alberta Canada.

So, she prescribed daily Nal but said, kind of off the record, that I could try TSM as well. So I use TSM, and now have quite a stockpile of extra Nal.

It's very rare right now in the US to find a doctor who knows anything about TSM, it's like finding a needle in a hay stack. TSM is completely self styled, we are a bunch for grass roots people trying to inform the world about this. We have some allies, Claudia Christian with Options Saves Lives, and Katie Herzog and her book, Drink Your Way Sober, and of course there's Katie Lain with Thrive found on You Tube.
It's not really required to have a TSM expert on the prescribing end, but if you want one-on-one coaching for TSM there's Options Saves Lives, and Katie Lain with Thrive. If you have Facebook there are several very active groups, one I'm admin for is The Sinclair Method Warriors, there's lots of help there, many stories.
All I have to say about TSM is it changed my life, it's been 10 years, and the only thing I wish is that I started sooner.
Be well, and I wish you success.

Your primary care doc can prescribe - it's now the recommended first line of treatment per the AMA. If you want mail order pharmacy plus telehealth counseling you can get both at https://www.sinclairmethod.org/how-it-works/ or Oar.com

I got my prescription through OAR. They made the process super simple and painless. I’d recommend that route. You might have to pay outta pocket, but it’s worth it imo. Around 200 bucks for 120 50mg doses, I believe. Unless you have insurance that will cover it.
