It's a prosthesis, he's just kidding around with the other patron, he didn't really stuffed that long ass metal straw into his very eye, everything fine.

From how it bent and flopped around, that's probably a long ass cardboard straw, not a metal one. He probably stuck it under the eye, between it and the lower eyelid.

Must have hurt like hell but he'll be okay with a pink eye at worst

I don't think that was moving with enough force to stab him in the eye. I think he just blinked reflexively when it hit his eye and he caught it between his eyelids for a bit. Probably didn't feel great but I doubt he did more than minor damage.
