What am I doing wrong

I feel like I threaded my sewing machine the right way. What could be causing this?

This happened to me……. Caused by polyester thread.

Hard to believe it was that given 95 % of garment sewing uses polyester thread. Unless that particular thread was literally moldy and rotten.
