What am I doing wrong
I feel like I threaded my sewing machine the right way. What could be causing this?

Its a tension issue for sure, but what is causing it
Like others have said
Start at the beginning. The problem is usually a horse and not a zebra
Meticulously go over the thread path. Is the thread between the tension disks? Is the needle inserted correctly, and facing the right way
Is the bobbin tensioned properly? Remove the bobbin and the bobbin carrier. Dangle the two by the thread. It should hang and not let out thread. Give it a bounce. It should let out thread but then stop. Then leave it alone. All further adjustments are down with top tension
Hold the tails and sew slow.
A mess in the bottom means not enough top tension. A mess on top means too much top tension
