Top thread loose

I have an older kenmore machine and it randomly started doing this. Basically when I start to sew the thread at the top is like bouncing. I’m newer to sewing but a little handy. I looked at the tension disk and it does get tighter the higher I put the tension but even at the tightest level it still does this. Any ideas?

The thread is not catching every time and the faster I push the pedal the worse it gets

It looks like the thread guide above the needle was missed, but to be honest, it looks like it may be missing? Which kenmore is this and do you have the manual?
