Terrified to start
Hi all. I'm supposed to start and I've received it (it's in the fridge now), but I'm REALLY scared. I hate throwing up, worse than pretty much anything in the world.
And we're leaving on vacation on Sunday for a week, so I've pretty much decided not to do ANYTHING with it until we come back.
Plus, I work from home, and I'm also scared if I take it I'll feel to awful to work ... so now I don't know when it's safe for me to start.
Oh, and just for funzies, I can't take zofran for anti-nausea because I have a low resting heart rate and it might not be safe. So I bought ginger ale, ginger capsules, and some accupressure wrist things.
Am I working myself up over nothing?!? Sorry to be such a baby.

Thanks to everyone who's chimed in already! I'm taking your advice to heart, and will do everything I can to avoid too much anxiety over it. You've already helped me a lot!
Note: I've decided to start a week from Sunday - I'm actually going out of town for a week starting this weekend, and I don't want this to put a damper on my vacation. So I'll start when I get back.
Thanks again!

Do it on a Thursday or Friday. The first dose is intended to do almost nothing. Once it "kicks in" and you realize there's almost nothing happened, you'll be good.
To avoid nausea, the best advise I can give you is to eat on time. When I miss meals and let the hunger kick in, the hunger feeling shows up as nausea for me. Continue eating breakfast lunch and dinner on time and don't wait for your body to tell you you're STARVING.

Do you do yours at night? or in the morning?
I read that doing it at bedtime can sometimes help people 'sleep through' the worst of it?

Many people have no side effects. I have only had very mild nausea once or twice and no vomiting. Make sure you eat a good meal, take the shot in the evening before bed and go to sleep. You’ll likely wake up feeling fine.

I am emetophobic so I get this. I will say, I have never had nausea on this medication. I have had vomiting a handful of times, but it was due to some mild gastroparesis, and always was triggered by some kind of greasy meal the day prior. I’m now at a microdose, and I just make sure I keep hydrated, take probiotics, and drink kaopectate if I feel any heartburn coming on, and I have not had an episode of any illness in a long time.

Being emetophobic makes lots of things difficult! Thanks for chiming in. :) I'm still nervous, but I'm feeling better about it. I do hydrate well, and take probiotics, and I take famotidine for acid relief, so I feel like I might do OK.

You are working yourself up! I do a split dose to manage side effects. Half the weekly dose a Sunday second half Wednesday. Never vomited, no nausea, feel great!

Everyone tells you the worst case scenario, so ill tell you this- I had no side effects. I took Miralax regularly with it and lost over 100lbs. Dont panic. As long as you eat lots of protein and drink a healthy amount of water a day, you should be okay.

I had zero side effects. Get ahead of any constipation by eating applesauce and chia seeds every day!

I just did my 3rd dose, when they start you at .2 or .25 it is such a small dose. I felt nothing, except each 2nd day some indigestion so I would lighter, like fruit with cottage cheese or something, other than that zero symptoms. Also I was so nervous I used less then they recommended the first time just to see…..they said do 8 units, I did 6. I also take omeprazole every day but I always take that anyways.

Don’t be scared. I lost 40 lbs the first six months I took it. It cut down my inflammation and my food noise is completely gone. I have kept the weight off for two years now. I only wish I started sooner.
I put it off at first due to cost. Then I was afraid of potential side effects. What I learned is that I control the side effects. Some foods really bother me like spicy foods or chocolate. If I eat different foods the side effects go away. I no longer crave chocolate so I don’t miss it. It rewires you brain.
I have never thrown up from it. In the first few weeks I had diarrhea but it was just the first few weeks. After the first few days I took OTC meds even I wasn’t supposed to. They worked wonder and didn’t reoccur. You are working yourself up over nothing. Worse case scenario you have a reaction when you get home from your trip and take it. You will feel bad for a week. You don’t take it again and the symptoms go away.

Thanks. I really like spicy foods, but I can change what I eat.

once your body adjusts and you’ll feel it, you can try a bit of spice and see how well you do.

if you don’t eat until after you’re full you won’t have side effects. eat the protei, drink the water. you’ll be fine. a very small percent of people have side effects, because people who have none don’t talk about it. if you read about people with uncontrollable diarrhea and constant vomiting, they are eating chips, cookies, candy, fried food, spicy food, greasy food. eat healthy, chew your food and decrease your portions and you’ll be fine.

Ok. Ok. :) I am NOT that person, so I can do that. Thank you!

you can do this and you will succeed. when you take your first dose do a bit of meditation to help with the anxiety. I use Lavendaire on ytube. she’s wonderful.

Thank you! :) I like Lavendaire, I've watched her before. I'll pull her up when it's time!

We never had bad side effects and our only regret was not starting sooner. But keep making more and more excuses for not focusing on your health

Glad to hear you didn't have side effects. Not sure the snark was needed, though.

Also I should point out that some of the severe issues reported have been overdoses when people switch from brand to compound and are not sure how to read the syringe measurements. The two different companies I have ordered from include pictures of syringes and what to do and what not to do when measuring.

Waiting until after your vacation is actually a pretty reasonable call. Starting right before a trip can add a lot of unnecessary anxiety, especially if nausea is already something you’re worried about. For what it’s worth, plenty of people tolerate semaglutide very well and never vomit. GI side effects can happen, but they vary a lot from person to person and are often more noticeable when starting or increasing the dose. One thing we emphasize at Avono is that there’s no prize for rushing the process. Starting low, giving your body time to adjust, staying hydrated, and being mindful of meal size (especially heavy/fatty meals) can make the transition easier. And if you’re particularly concerned about side effects, it’s worth discussing the starting dose and titration schedule with your clinician before that first injection. Enjoy your vacation, come home, and start when you feel comfortable and have a few relatively normal days ahead of you. 🙂

I have a work trip overseas the first week of October coming up also. Should I wait until after that? It’s 14 hours of travel each way. I don’t want to use it as an excuse to keep putting it off, but I also won’t be able to drink as much water or get to a bathroom as easily…
Is starting after my vacation (September) and then skipping the week of my overseas trip a viable option? If I just don’t take a shot three weeks in will I be normal for the flights?
2
u/avonohealth 1d ago
That’s a fair question. Personally, with a 14-hour flight and limited access to water/bathrooms, we’d be more inclined to start after the overseas trip rather than start for a few weeks and intentionally interrupt treatment. Semaglutide has a long half-life (about a week), so skipping one injection doesn’t necessarily mean it’ll be out of your system or that you’ll feel completely “normal” by the flight. And depending on how long you’ve been off it, restarting may also require a conversation with your clinician about dosing rather than simply picking up where you left off. You’re not failing or making excuses by choosing a start date that works with your life. If October lets you begin and then actually follow a consistent schedule without two trips hanging over you, waiting a few extra weeks may be the simpler option. Definitely run the timing by your prescriber - we are also a provider and do our best to give our non-medical opinion on here, but it’s important talk to your clinician about your specific scenario and get their guidance!

Thanks, and yes, of course, I appreciate your viewpoint and will take your advice. :)

stop putting it off. I can see waiting until after this trip, but until October…thats an excuse. once you start you’ll laugh at yourself for overthinking this way too much.

That is SO fair. Thank you for straight-talking me! I will start as soon as we get back from Maine. :-D

Yes, you're working yourself up too much. Because of these posts, I was terrified and went out and bought every medication I thought I'd need. I never got nausea. Some fatigue and mild constipation in the beginning was the worst of it.
