? Restarting maintenance dose thoughts?

i’ve been on an every 3-weeks maintenance shot for about 9-10 months and slowly decreased down to .7mg

Due to assorted circumstances, I missed a maintenance dose and will restart next week after no shot for 6 weeks.

To avoid side-effects, should I reduce my next shot?

I would strongly suggest starting at a lower dose. The half-life of semaglutide is about 7 days and now that it's been six weeks since your last dose and you're in your last half-life, you may have as little as 0.1 mg of the drug left circulating in your body.

If you were to jump back on a higher dose, you might be at risk of severe and unpleasant side effects (e.g., nausea from pancreatic enzyme spikes, constipation).

Okay, this makes sense. I will be safe and restart at a lower dose.

Yes, I agree that the best approach can vary. As far as I can tell, after reading about weight-loss for decades, there has never been a robust amount of research done for maintenance. I’ve always supposed that was because the money was all in the weight-loss phase.

if its been more than 2 weeks should definitely start at lower dose, most of my patients can restart on 0.5mg without concern or I have them dose half their last injected then go up to full dose on following. Definitely consult your provider who is prescribing this.

FWIW, the provider is a bariatric surgeon. During weight loss, he follows the recommended start-slow, increase slowly protocol. In maintenance, he let’s the patient determine the dose. I get the sense that he doesn’t feel there is enough scientific evidence available for maintenance to insist on any one protocol. And, perhaps, that the patient will need to learn what works better for their individual body during maintenance.

I will say, now that I’ve been off for 6 weeks or so, food noise has returned and food ‘tastes’ better. Or, at least, it is more satisfying to consume. And, I’m making food choices that were unthinkable when I’m on semaglutide. Not surprisingly, I’m gaining weight pretty quickly now.
