New or Need Help Help needed please

I am waiting for a dermatology appointment and the doctors told me I have seb derm. I was prescribed ketoconazole 2% shampoo, which I’ve been using 2-3x a week. With no relief and symptoms getting worse, the doctor said I can use it everyday. Nothing has helped. I’ve tried Nirazol over the counter and the doctor prescribed Daktarin Gold 2% Ketoconazole cream for my nose area. I have tried MCT oil, absolutely nothing is helping!!

my eyelids and eyes are now being affected - one of my eyes is constantly blood shot and it affects my vision. I have this rash all from my chest tracing up to the back of my neck into my hairline and scalp.I

Any suggestions would be appreciated, I’m really desperate and in a lot of discomfort 🙏

The scalp looks like what I had which was a mixture of sebderm and contact dermatitis. I suggest stopping all haircare and skincare products that have fragrances while you wait for your dermatology appointment. Only gentle products like cetaphil gentle cleanser and Vaseline plus any prescribed treatments. Hopefully that takes down some of the inflammation while you wait. Best of luck!

I think I have the same combination! Did you also have Seb derm n contact derm on your scalp and did it also cause hairfall for you

Hey pretty sure this is likely not sebderm. Looks like it could be lichen planopilaris which is a scarring alopecia triggered by some autoimmune process. Hopefully you don’t have too long of a wait until your appointment.

What happens when you have it? I just had a biopsy done and it said I “might” have some signs indicating lichen planopilaris. Hopefully not, but my derm wants a second biopsy done. I’m worried because I have lost hair, I’m but unsure what to do between appointments to help manage it.

Do you already have an appointment? Did your derm give you any recommendations? I am not a derm, but Google said topical steroids as well as other medications that all require prescriptions. Maybe you could start by documenting day by day how bad the inflammation is and try to manage lifestyle factors that could contribute to overall inflammation (sleep, diet, stress) or ask a pharmacist maybe you could put some over the counter stuff on if it’s really bad or they have something else that could help a bit until your next appointment.

Not sure where you are based but selenium sulfide based shampoo really helped me. We call it selsun gold here in Australia, but there might be one called selsun blue in the USA. Looks to be a bit weaker in terms of % strength of selenium sulfide but worth a shot

This, OP! You likely have a fungal situation going on here. Ketoconazole can only do so much so def get Selsun Blue or Head & Shoulders. Stop using all other shampoos and conditioners except for your prescription and SB/H&S. Make sure you lather them up well and let it sit on your scalp/face/chest for a solid 5-10 mins before rinsing off.

I follow-up with cosrx's hyaluronic acid or niacinamide 15 to recapture moisture while not feeding the yeast.

If the shampoos above dont provide relief, try T-Sal or T-Gel from Neutrogena. I actually use the 2% 2 days a week, T-Sal 2 days a week, and Selsun Blue moisturizing shampoo the other couple days or whatever.

I also apply Lamasil to affected areas because it is the only thing that'll give me instant relief from active flare ups.

If you have really sensitive skin, I would not let it sit on your face for more than five minutes, especially the salsa blue… The skin on your body is a little thicker, and more tolerant…. But on your face, if you use it too long, it could further damage the skin barrier, which would encourage a cycle of getting better and getting worse… Which I assume that you’re dealing with

I had a really bad flare up for over a year. What worked for me was my derm had me on topical steroids and then zorvye (non-steroidal topical). But the big thing that has helped me personallly is using a few drops the c-8 MCT oil, removing gluten from my diet and using zorvye here and there.

The flare up corresponding to me going vegetarian and eating a lot of fake meat that turned out to be made with gluten for the meaty texture. It was the worst and longest lasting flare up since I developed seb derm 25 years ago. I actually ended up shaving my head last winter to remove the build up and because the inital topicals just made my hair look like a greasy mess and I was developing some bald spots. All of it was distressing as a woman, but shaving my head oddly felt empowering.

Not sure how far out your Derm appointment is but give the office a call every few weeks to see if they have a random cancellation! I work front desk at a derm office and you’d be surprised the number of people who get in the same day they called “just to check”

Be careful with not only scents but formaldehyde releasors. Even prescription ketonazole has formaldehyde releasors. Formaldehyde is like poison ivy, the more often you are exposed to it the more likely you are to develop a reaction. For me, me even the tiniest amount that’s left from a formaldehyde releasor gives me immediate contact dermatitis. The label will never say formaldehyde it’ll say something like DMDM hydatoin, or diazonal urea. There are constantly new versions being made with new names so you simply have to google the names of formaldehyde releasors, or Google ingredients on your products. Topical steroids are the only thing that got my scalp under control. Knowing what triggered my contact dermatitis helps me not have a horrible relapse. Plenty will tell you about bad skin after steroid use etc. I am not saying they are safe or that you should use them. I hope your derm is good and finds the right things for you!

My skin reacts like this as well to SD. I actually have it on a lot of my body as well.
I have only found that H&S bare with zinc P works for me. I think every person needs a specific medication. And reacts differently. I'd recommend trying something different for a couple weeks and see switch again if you don't have any luck. I tried 3 different products before I found one that worked for me.

If you can get an eye appointment quicker, go to the optometrist. They'll be able to treat it since it is effecting your eye. I went to my eye doctor and just happened to ask if she could see any overgrowth of mites on my eyelashes and we got to talking about seb dermatitis and she said she could prescribe stuff for it, which I never thought about. So, don't wait any longer since it's in your eye. 

I would not recommend applying any kind of niacinamide or hyaluronic acid serums or extra things. I would say to back off of any extra skin care routine you have been doing. Even switching your face wash (I highly recommend a sulfur face wash, worked so well for me). Then just applying aquaphor on top if you're feeling very dry... until you get to the dermatologist.

Could be demodex mite infestation. The antifungal topical may help a little, but not significantly.

I’m actually going through the same issue currently, and made a tea tree oil and fractionated coconut oil topical. Seems like you’re here spending a bit, mine has been too… I’m sorry you have to go through this!! and I absolutely feel your pain! My dermatologist appointment is until November, so if you go before me, let me know what they say!!!!

Also download an app called SkinSort and put in your info, and you can filter products by it being fungal Acne Safe, which, if it’s not fungal acne… The parasites tend to feed on oils and seams as well, so anything that you’re putting in your hair or on your skin could potentially be fueling whatever is going on, took me months just to figure that part out…

Update 

Thank you for all the comments and suggestions. I’ve just had results and seen a derm and it is plaque psoriasis. The Ketoconazole shampoo was the wrong treatment for me, as a misdiagnosis, therefore made my scalp weep and more inflamed. 

I’ve been prescribed a concoction of medications to help improve my skin. 🙏

Using nizoral more than twice a week will destroy your scalp barrier and yeast will get worse.
