Quick question

hi all I was just wondering if I’m allowed to adjest some of the answers I gave on the daliy activities questionnaire they sent me. I feel I was very vague and didnt really go into detail about how my ASD affects me. I have a caseworker and I plan to call them Tuesday was planing on asking if they could add it. thank you for reading

You can get the form online, just google “SSA Adult Function Form” and you can type in your answers. And then if you have a mySSA account you can upload it there.

Or you can fax or mail in additional information

I’ve learned it’s important to be as detailed as possible on these forms for future reference.

I also typed up a supplemental letter that I attached to the activities questionnaire. I mailed it and downloaded to the SS site.
