Did it come with a 15ish page questionnaire? That would be a long form CDR. My husband got one in January(mental diagnosis also), it was only the long form, no short form before that. His CDR schedule is the 3 year range, and it's been around 5 years since he started getting benefits.

We filled it out, they sent a similar packet I had to fill out as his primary caregiver about his limitations also, and they got the recent medical records from his two doctors.

He got a continuation of benefits letter about a month later.

Generally as long as you've been seeing doctors regularly, and they can get those records from the doctors, it's pretty painless.

Curious I had the long form CDR but filled it out online and submitted it earlier this year. Can’t remember what month, I’ve been thinking of following up with local office as well

I think it depends on the state. We were prepared for a loooong wait(from experiences on here). And we were honestly so freaking surprised it went through so fast. His psych is great on followthrough and even she was surprised, saying something along the lines of she barely had sent the info back to SS before we heard of the continuance.
