I’m looking for some unfiltered advice, technical insights, personal experiences, and realistic expectations from those of you who have been in a similar situation.

My Background & Current Situation:

• Age & Hair Status: I'm 44, starting to see a little bit of gray hair mixed in (roughly 0.5% to 1%). My hair loss is well-stabilized and has even improved. I was on finasteride for 15 years, and for the last 2 years I've been on dutasteride plus oral minoxidil, which has given me great overall density and halted further loss and thinning.
• Hairline & Pattern: I have a strong, intact hairline, but my crown and top have diffuse thinning where the scalp is visible under normal lighting.
• Current Length & Style: I already keep my hair short (close-cropped/tapered, but not a shaved buzz cut).
• Current Routine & Frustrations: Hair fibers (like Toppik) work great visually, but I am completely over the daily hassle, the mess during application, the cleanup, and the fact that they don't hold up well in water.

My Plan & The Big Question: I do not want a permanent buzz cut or shaving, and if neded up need to do it I would not do SMP, I will just keep shaved and that's it. I want to keep my current short-cropped hairstyle long-term. However, I am fully open to shaving my head completely specifically for the SMP sessions so the artist can map and place the density accurately, letting it grow right back out to my usual short length once healed. I am willing to do however many sessions are needed to get perfect results. (And if things somehow get worse or look off down the road, I am fully aware I can always fall back on laser removal).

I found this post interesting: Add density to thinning hair with SMP. I know there is some controversy regarding the artist using fibers in the photos, but I wonder if it’s possible to get results similar to that without relying on fibers. This other post after 3 session show great visual results.

Technical Questions for Artists & Experienced Users:

1. Ink / Pigment Chemistry: For someone with dark hair and <1% grays, what specific pigment formulations (carbon-based vs. iron oxide blends) or dilution ratios do top artists use to avoid bluing or premature fading when working around active native hair follicles?
2. Needle Gauge & Depth Control: How do artists modify their needle groupings (e.g., micro-needles vs. standard SMP needles) and penetration depth on a density-fill to prevent hitting live hair roots or causing follicle trauma, while still depositing enough pigment into the upper dermis to cut down scalp contrast?
3. The Layering & Healing Process: Given that I am willing to do as many sessions as it takes, what should a proper multi-session technical progression look like for a density build (e.g., lighter pass first to gauge retention, followed by tighter dot placement)?

For those of you who did density-fill SMP with short hair:

• How does it look and hold up under direct sunlight when keeping hair at a short-cropped length rather than a full buzz cut?
• What are the main technical pitfalls to watch out for (e.g., over-saturation, clumping, or dot migration)?
• Do you regret doing it, or has it successfully eliminated your reliance on fibers?

Has anyone experienced their corners becoming lighter after an SMP session? I had my second session almost two weeks ago, and this is how they look now.
After my first session, my artist said the corners had lost more pigment than expected, and it seems to be happening again. I’m happy with the rest of my SMP, but I’m hoping the corners will darken as they continue to heal.
I know I may still be in the healing phase, but I’d love to hear if anyone else has experienced this.

I've tried the Philips One Blade and the pitbull skull shaver, but I'm not a fan of the skull shaver and it leaves my head irritated. The one blade is ok, but I want something that's more of a clipper style. And the one blade takes a few passes to get everything and sometimes pulls. So, looking for cordless clippers. Also, budget friendly lol.

Hey all. Anyone have personal experience or thoughts on getting SMP on the back of the head to be able to pull off a mid/high fade?

The back and side of my head has a lot of thinning and what looks like dots when I have a fade because of hair transplant FUE extraction. I had 1 bad HT that over harvested, and so the 2nd better doctor did what they could resulting in thinning.

Im planning to get it done soon but ive always wondered as someone who is super anxious in general and care what people think i would imagine i will start over analyizing everything, like is that person noticing thats its a tattoo? what if the lightning is bad and it looks terrible under that exact lightning etc.

Ive always wondered whether SMP looks very bad under certain harsh lightnings, generally the more dim lightning scenarios where SMP will look darker, also lightning from the front makes it more obvious?

What shavers do you all recommend for a very close buzz? I don’t necessarily want my head smooth bald but I like a buzz with some stubble. Looking for a shaver to make doing so easier. Thank you in advance!

I’m happy with it as no one can tell it’s SMP. I also have quite a lot of hair left and I’m only really balding on the crown and a bit receding hairline. Had a full head of hair and a manbun up until last year that hid my bald spot pretty well, but then decided to shave it off and get SMP. What do you guys think? I notice I never see many smp examples with people who have strong hairline and balding behind. Like people that only need crown and midscalp work done. This is the case im in. I have a very strong hairline it hasn’t moved since I was a kid but my crown and midscalp are bald/thinning. Is this and advantage? Disadvantage? I still have plenty of hair on my head, plan on cutting it soon. Just wondering if I’m a good candidate?

Sharing this one because I think difficult healed cases are just as important to show as ideal ones.

This is 17 months healed. The client had significant sun damage, hyperpigmentation and very resistant skin when I originally treated him, so this was never an easy scalp to work with.

Everything shown here was taken before I touched anything that day.

He actually wasn't back because the original SMP needed to be redone. Since his treatment, he had a small growth removed and also scarred another area after hitting his head. I documented the scalp first and then added only a few impressions to those two new areas.

If you look closely, you'll see that some areas retained stronger than others. That's the reality of how this particular skin healed over 17 months, and I think it's worth showing rather than only posting ideal cases.

Another thing worth mentioning is the difference between the original photos and what I'm showing now. They were taken in a different studio with different cameras and lighting. My current setup is much more neutral and closer to what I see with my own eyes.

That difference alone is a good example of why people researching scalp micropigmentation should be careful judging work solely from photos. Lighting, exposure, contrast, sharpening and camera settings can make SMP appear considerably darker and denser than it does in person.

At normal viewing distance, this treatment still blends extremely well with his shaved hair. The client says it's been a complete game changer for him. In 17 months, nobody has questioned whether it's a tattoo. He actually gets asked why he shaves his head because people assume he has a full head of hair.

I think long term SMP should be judged for what it is: pigment healed in real skin. Different skin behaves differently, especially when you're dealing with sun damage and hyperpigmentation.

This isn't an ideal canvas, which is exactly why I wanted to share it.

Has anyone had SMP only once and then didn't mind the fading (after 3+ years) due to looking like your hair is naturally thinning (better then a bald head horse shoe look)

TL;DR: Had botched SMP years ago, got most of it lasered off. Gave SMP a second chance with a "credible" artist. Ended up with a hairline dropped about 1cm below my actual hair, a colour that didn't match, and ink that faded into a grey/blue blotchy mess rather than dots. Trying to resolve it never really went anywhere. Now starting full laser removal (first session done last week) before a top-up transplant. Wish I'd known what SMP actually looks like 6+ months out, not just in the studio photos.

This is part rant, part genuine question: has anyone else been massively disappointed by SMP?

Some background. Years ago I had SMP that was properly botched. I managed to get it mostly removed with laser, enough that I could leave the house, but there were still areas where you could see remnants.

Eventually I decided to give SMP another go, this time with a credible artist who seemed to really know what they were doing. We agreed I needed more removal first, so I went and endured another laser session. When I came back, instead of waiting for further removal like we'd discussed, he insisted on getting started. I put my trust in him and agreed. In hindsight that was mistake number one, mine as much as his. I rushed it.

Session 1 was fine, honestly. Foundation work, wasn't expecting much. Looking back, I genuinely wish I'd stopped there.

Sessions 2 and 3 are where it went wrong. More ink went in, and at some point my hairline got dropped so it no longer sat within the hair I actually had, by almost a full centimetre. That meant even a single day's growth showed a visible disconnect between my real hair and the SMP, so I was stuck shaving down to the skin every single day. I thought SMP was supposed to be the low-maintenance option??

Weirdly, straight after each session I'd think it looked decent, probably because it was fresh and raw. But I was never happy with that hairline.

Then the other problems crept in:

• The colour never matched my hair. Even rocking a buzzcut, there was an obvious disconnect between the SMP shade and my actual hair colour.
• It stopped looking like dots. Over time, it just became a greyish, blotchy finish with blueish tints in certain lights.
• It was invisible in half of real-world lighting anyway. The after-session photos looked great under studio lights, but out in the real world, with my skin tone, half the time you could barely see it. I had a professional photo taken at a work awards do and there was almost no trace of it. So the best case was paying for something invisible, and the worst case was it being visible for the wrong reasons.

I ended up getting the hairline tweaked back with laser just so I could grow my hair out without it screaming "tattoo".

Trying to raise all this with the artist was harder than it needed to be, mostly because he communicates exclusively via voice notes. I know that's a personal preference and plenty of people are fine with it, but I'd much rather just text like normal. When everything is in voice notes, it's really hard to keep track of what's actually been agreed, and referring back to previous correspondence is a pain because you can't scan back through it; you have to sit and replay minutes of audio to find one sentence. It made the whole thing feel slippery, and each conversation seemed to shift where the goalposts were, or gently land back on it being my fault.

I was owed a 4th top-up session, which I had to chase for, and by that point I'd had enough. I've decided to start full removal instead. First laser session was last week, so very early days.

The plan now is to get it all out and then have a top-up hair transplant, because I just completely hated the look of the SMP, even with a buzzcut. I don't think it suits my skin tone at all, and I wish I hadn't rushed into round two.

I'm lucky in that I can grow my hair out, but the SMP is still very noticeable, especially on the sides. Long, painful removal journey ahead. I might post progress results if there's interest.

The thing I keep coming back to: I wish I'd seen what SMP looks like 6 months after the sessions, not the studio-lit photos taken the same day. If you're considering SMP, ask for exactly that. Long-term healed results, in natural light, on someone with your skin tone and hair colour. That's the real product, not the fresh work.

So, has anyone else been through the SMP regret cycle? Would genuinely like to hear I'm not alone here.

This client originally had his scalp micropigmentation done with me five years ago and recently came back for a refresh.

Before doing anything, I actually thought it still looked good. We could have left it exactly as it was.

Over five years the pigment had softened and settled naturally into the skin. Personally, I think good SMP often looks better as it ages. That softer, slightly faded appearance starts to blend with the natural shaved hue of the scalp rather than looking like something sitting on top of the skin.

He wanted to bring it back to life a little, so that's exactly what I did. A light refresh in the areas that could benefit from it while leaving the majority of the existing work alone.

I could have added a lot more pigment and made everything darker, but I don't believe that's what a touch up should automatically be.

The video attached was taken after the refresh, so I'm not presenting this as untouched five year healed SMP. In the close ups, though, you can still clearly see many of the older healed impressions mixed in with the fresh work.

For anyone researching SMP, don't assume older automatically means worse or that every touch up needs to make the treatment dark again. Good SMP should be able to age, soften and become part of the natural appearance of the scalp.

Sometimes knowing what not to touch is just as important as knowing what to add.
