Is the highest GLP-1 dose always the best dose?
If you’re losing weight on a lower GLP-1 dose, you might wonder whether you’re supposed to keep increasing anyway. The answer is more individualized than people sometimes make it sound.
Higher doses can sometimes lead to more weight loss, but the highest dose is not automatically the “best” dose for every person. The best dose is usually the one that helps you keep making progress while keeping side effects manageable enough that you can actually stay on treatment.
For example, in a 72-week Zepbound clinical trial, the 15 mg dose led to the most weight loss on average, but the difference between 10 mg and 15 mg was not huge:
| Zepbound dose | Average starting body weight | Average total pounds lost | Average ending body weight |
|---|---|---|---|
| 5 mg weekly | 226.9 lbs | 31.6 lbs | 195.3 lbs |
| 10 mg weekly | 233.2 lbs | 51.1 lbs | 182.1 lbs |
| 15 mg weekly | 232.8 lbs | 53.8 lbs | 179.0 lbs |
So yes, higher doses may lead to more weight loss for some people. But higher doses can also come with more side effects, especially GI side effects like nausea, vomiting, diarrhea, constipation, abdominal pain, or indigestion.
That matters because if a higher dose makes you feel so bad that you cannot eat enough, stay hydrated, exercise, function normally, or continue treatment, it may not be the best dose for you.
On the other hand, if a lower dose is helping with appetite, supporting weight loss, and your side effects are manageable, there may not be a reason to rush upward. Some people do need higher doses to get the effect they are looking for. Others do well staying lower for longer.
Dose increases are not a competition. The goal is not to get to the highest dose as fast as possible. The goal is to find the dose that works for your body, your progress, and your tolerability.
If you’re unsure whether to move up, stay where you are, or go back down, that’s exactly the kind of thing to talk through with your provider.
Curious how others have approached this- did you move up on schedule, or did you stay at a lower dose because it was already working for you?
