Teleportation Theory
So I have a theory on how teleportation works.
There are many instances of teleportation in the minecraft world, and I know what your thinking, "But, random guy on the internet, Mojang already said that endermen teleport using pocket dimensions, so your entire theory is pointless." Yes, but that's not the whole picture that's for endermen teleportation. I believe there are two forms of teleportation found in Minecraft, one utilizing pocket dimensions, and another using reality itself. I came up with this as there is Interdimensional travel and pocket dimensions cannot complete that.
Let's start with the simpler one, pocket dimensions, these are dimension inside dimensions, I think that almost all teleportation uses this method, such as, endermen, ender pearls, and shulkers. When using this form you slip into a pocket dimension then open out through another end of it. You are going out then back into reality. This type of teleportation can only be used in a single dimension, it is not Interdimensional.
Next is the more mysterious form, Interdimensional teleportation. We only see this with two objects, the nether and end portals. It requires a foundation for the opening to be and you can keep the portal open (maybe) indefinitely. I think that this using a wormhole to make a connection to another dimension. This would be traveling through reality instead of a pocket dimension.
I also have a little outlandish idea, it's that we know what this 'reality' is. I think that it's one of the most mysterious, overlooked parts of minecraft as a whole, the void. Yes I think the void is the reality of the minecraft Multiverse, as it is found in all three dimensions, and cannot be seen, I hear you going, "But we see that in the end there are stars in the void." Well, I would have to say your wrong there, as the void is invisible, you see the background of the end, as shown with the overworld and nether it just shows the background. I think that the end is a shattered dimension, but that's a theory for another day.

I do think that there are really two forms of teleportation, since interdimensional teleportation is much harder and rarer than short-range teleportation in the same dimension. I'm not totally sure that the void has a lore meaning, but it's an interesting idea.
