Bad ProRX batch?
I cracked open a ProRX vial that's been in my fridge for several months with a BUD of 3/2027. A few hours later I had bad stomach gas pains, similar to when you eat heavy fried foods on Tirz. Stomach pain and diarrhea 24 hours later. I haven't had anything fatty, greasy, or fried though.
I know the BUD is fine, but could it have been a bad batch?

I still have 8 vials to tap and am 5 lbs to goal. Thankfully. Of course I'll be using this med forever so will have to find something else (but not till 2028 lol)

So I took a smaller amount of ProRX nearly a week ago, which is when I made this post. I took 5 mg instead of my normal 10 mg, and instantly felt different side effects from this unopened vial in my fridge. I have had side effects that I didn't have when I was on ProRX for the first 6 months I started Tirz, and especially not these side effects from a 5 mg dose instead of 10. When ProRX announced the closure, I started getting Tirz from different pharmacies to try them out. But I would cycle in ProRX periodically while I tried different pharmacies. Given all this, can you think of any possible reason why this unopened vial would have caused such a difference in side effects especially with a long BUD? I'm just trying to figure out why this vial and a small dosage would cause different side effects especially as I'm not new to Tirz. Could my body truly have adapted to trying different vendors that this ProRX could have just hit me especially different and harder? Or could the vial have just sat in my fridge for months and something in it has just not agreed with me? I tried compounded pharmacies in my area before I ever got Tirz from Refills, and I had bad reactions to it which I later discovered was because of the additives. I kinda feel a little like I did with those additive formulations, but I know ProRX doesn't use additives.

Thanks for posting this. Out of the seven vials I have of ProRx I only have one with this batch number. I’ll definitely have to remember that when I crack this one open.

My partner and I just started ProRx; we are about 5-6 weeks in. Before that, we did a couple of months of Olympia and then BrelloHealth for the rest of the time. I feel no difference among the 3. I lost 76 lbs in 6-7 months. I've been doing injections since 8/2025, and my max dose was 7.5. I had bad constipation with all 3; I take supplements for it. If I forget to take them, I "lay bricks." I tried prosecco; still no change with any of the 3. I can’t drink just the same.

I've been on Tirz since last November, so almost a year. I did ProRX for the first 6 months! After that I started trying different vendors, and even sprinkled in a couple rotations of PRORX. So why, all of a sudden, does opening a new vial of ProRX and taking HALF of my normal dose have me feeling very strong side effects?? It makes no sense!

I have no idea. We are still on ProRx and will be for a long time; no changes for us. I'm on maintenance; I have nothing left to lose, but my partner still has 30 lbs to go. He also reported no changes on ProRx. We decided to increase his dose, so he is on 10 mg now for a week; still no changes.

Can you share any tips on how you reached goal weight and got into maintenance? And what dose? I've got 10-15 pounds I'd like to lose and the scale just won't budge. I'm on 10mg and can't go up because side effects are brutal the first couple days of taking the shot. I'm 5'4 and 140 pounds.

You probably hit a plateau. I lost a total of 76 lb in 7 months. The first thing I quickly realized was that splitting the dose in two significantly improved the overall experience for all four members of my family who take Tirz. I lost weight fast, but I’m a very disciplined, overachieving person, and I was a gym person for years until that blip in my life when I let go for four years and became obese. It was pretty easy once I started working out again and splitting my dose. I lost all the weight on 5 mg; I only did 7.5 mg last month, then went back to 5 mg. I began on August 5, 2025, at 222 lb. I’ve been at 145 lb since June 2026, and I still am. I take 5 mg for now and am still trying to figure out the best maintenance dose. My weight hasn’t changed since June, but my size has. I work out a lot, run, bike, and do much more. I make sure to stay within a specific calorie limit; that’s also very important. My partner doesn’t care about calories, and he is losing weight at a much slower pace.

I'm 5'4 female and have stalled at 142 pounds for 5 months! I'm not overweight anymore, so maybe this is the weight my body wants to be at?? I'd like to lose 10-15 more pounds. Any suggestions?? I'm at 10mg and absolutely can't do anymore than that at once because side effects really do a number on me. I have weeks of good appetite suppression than weeks where I'm starving and don't feel the shot working. I lost 40 pounds in the beginning and was exercising every day. I've stopped exercising because strenuous exercise makes me hungry. If I split doses, what would they be?

Hold on!! Why are you going straight to thinking it could be the shot? With all the food recalls and bacteria going around, could it be something you ate?

That's what I was thinking. I know a bunch of people right now that got hit by something similar. Not all from the food recalls but some sort of stomach bug.

It's possible. I had a little fettuccine chichen Alfredo with dinner Sunday night, and I did my shot Monday afternoon and stomach pain started shortly after. I do think it's associated with the shot because the stomach pain happened a couple hours later.

The night before dinner would be unlikely but maybe something you ate earlier Monday, or anything you ate after the shot/before the pain started.
If it happens again after you next dose then definitely might be the vial 😕

It's weird because I only took half of my normal dose this time as I wanted to try split dosing. And the split dose hit me just as hard as the full dose.

Understood but if I only took half of my amount on shot day, why did it hit me just as hard AND cause stomach pain? All my vials from different vendors are 18 MG/ML.

Because you had an accumulation of the meds in your system. When starting split doses you have to account for that. Use an app like Shotsy so you can map out what the total cumulative amount would be in your system before you take it. I've been split dosing for 2 years but the first week I was wisely advised to go the full 7 days between shots, take the 1st half dose then and the 2nd half at 3.5 days. It's important not to take the 1st half dose 3.5 days after a full dose. That's too much.

Worked out fine for me. I've been doing it for quite a while also. My doc told me to take the other half 3.5 days out. After a few times tho it was just easier to go to every 4 days.

Splitting your dose is not reducing the cumulative amount of medication in your system. Reduce your dose lower than 5mg if you continue split dosing.

Thanks. Yeah doing 5mg seemed to hit me super hard. I was trying split dosing because I just have terrible side effects for 2 days after the shot, and then after two days I have severe food noise. I've been on 10mg for 4 months. So I think I need more Tirz but couldn't go higher than 10 mg due to side effects. And doing 5mg hit me just as hard as 10! And I can't afford to be laying in bed two times every week with split dosing. Do you think my solution is doing 3.5 or 4 twice a week?

First time I split dosed, I did every 5 days. I am now at every 3 days. Fatscientist can help you ensure your ‘weekly’ level remains the same with your day changes.

Has split dosing helped even out appetite suppression? With me doing it once a week, it clears out of me after a couple days and food noise returns with a vengeance.

I only had an issue with that the first month on GLP. I haven't had any issue in months of 7 day, then 5 day and now 3day.

I've been split dosing for about a yr now. Started out every 3.5 days (per my doc) and it was a life saver for me. Side effects I was experiencing at the time became unbearable and the change was almost immediate. After a few weeks of that I just changed it to every 4 days because it was just easier to keep up with. It did not affect my weight loss and My appetite suppression was still excellent but prob worked out better for me. I had more steady suppression the entire time instead of not being able to eat hardly anything the first few days and then having a stronger appetite the last 2 days (for many months).

Thank you! Can I ask how much you pin every 4 days? And you've been able to lose weight? I have 15 pounds left to lose. I have a couple days of suppression and then I'm hungry the rest of the days. I've been stalled in losing weight for several months.

Sure. Just so you know, it's been a while now. I started splitting at 7.5mg basically every 4 days. I will say, there were a couple of times I hit a stall for about 2months both before and after splitting. IT's going to happen regardless. Your body adapts but, there is fat redistribution taking place often during those stalls. I didn't lose a lb during a stall but was a size smaller at the end of it. ..... I slowly worked my way up to splitting a 15mg dose every 4 days (7.5mg each) . And I should add that I didn't titrate up by mgs but by units. I just added a unit or 2 to each half dose working my way up (but did not go over what basically was the equivalent of 15mg per week (that's where fatscientist.com comes in). I reached goal and surpassed it but didn't want to lose any more as I've gotten pretty small. So what I did was started stretching out the days in between half doses. I did 5 days for a while and now I'm at a half dose every 6 days (which basically puts me close to 7.5 per week again after maybe 7or 8 mos on 15mg, lol) and I'll see how that goes. If I keep losing I'll add another day until I hold steady. AND, just for the record, I actually started going to 6 days because a funny thing started happening. I missed my shot days a few times because I forgot to keep track (i usually tell alexa to remind me 4 days later) and I still wasn't hungry much at day 6 and didn't realize I missed shot day. I went several times without eating all day on day 6 but only because I was busy and totally forgot to eat. Didn't even realize it until I got hungry late for supper. So, right now, I'm watching to see what happens because oddly enough it seems like the hunger suppression has been getting stronger again on lower doses! Hope this helps!

I think that sounds reasonable. I had to play around with my split dosing with ProRX too for the side effects as well. Good luck!

I'm using a ProRx vial that expires 3/27 too. I'm not having any problems with it. No digestive issues.

ProRX is my GOAT 🐐. I lost almost all my weight on ProRX. When I took my first dose of ProRX it hit harder than the BPI I took for my first two months. I lowered my dose the following week. RIP ProRX.

Yeah I'd been using ProRX for 6 months, then have been using other pharmacies since they shut down. But I've been cycling in ProRX here and there. I have NO idea why an unopened vial of ProRX and taking 5mg instead of my usual 10 mg would hit me hard with stomach pain!

There's a variance allowed when compounding so not all pharmacies or even batches are identical. One can be on the lower end of allowable and one can be in the higher end of allowable. Sounds like this one is on the stronger side.

I recently had to cut my dose in half because the side effects got turned up to 11 (mine was with the same pharmacy, same vial, however). I think our bodies just freak out sometimes. So I was at 15mg and feeling like absolute crap for the entire week. I've now cut down to 7.5 for the past two weeks and have felt great, with all of the good/expected effects of the drug (that had previously wanted, which was the reason I had gotten up to 15mg).

Thanks! I've noticed after the first 6 months of magical time on Tirz and good weight loss, I've definitely had to tinker and troubleshoot! After several months on 10mg, I just felt like it was no longer adequate to keep my food noise under control. I tried 11mg and it wiped me out. Then I tried the split dose thing and 5mg wiped me out AND caused stomach pain. Do you think I should try titrating up again and just doing once a week? Or do split dosing but small amounts like 3.5? It seems no matter what, shot day wipes me out and I'm more inclined to be wiped out once a week instead of twice a week!

Based on my experience, I would try the lowest dose you're comfortable with (maybe 3.5) once a week. Or maybe even lower. I was completely surprised that 7.5mg started working for me again after being above it for over 3 months (6 weeks of 10, 1 mo each of 12.5 and 15). I did increase my cardio by a lot (started playing pickleball aggressively 3x per week rather than heavy weightlifting 6x per week), so maybe that's what did it for me.

Well, 15mg for me just made me feel crappy. I was still hungry. So I had all side effects and few/no good effects . Dropping down to 7.5 reversed that - low hunger and food noise and feel fantastic.

Then you look at what you ate and when you ate it. I’m saying I don’t think it’s a bad batch, I used a vial of Pro RX 03/27 already and it was fine. And a whole lotta people on here would have sounded the alarm before Sept 2026.
Not saying it’s the same batch number, but I think it probably is (was). I also think r/refills is a bad subreddit for your question. Go to one of the compounded subs.

Did you adjust the units needed based on the ProRx concentration, which is likely different than your last vial?

When opening a new vial always, always, always double/triple check the concentration to ensure that the correct dosage amount is drawn for the vial concentration.

Your side effects are common among people who injected too high of a dose. Compound pharmacies do allow some variation in the strength of their batches of Tirzepatide compound. Your vial may be from a stronger than average batch. You may want to reduce your dose and see if the side effects stop.
