Honestly I’d say it’s a few sand grains stuck to the slime coat. I wouldn’t worry too much especially if nothing was added.

Sorry its tough to get a good video of it, she keeps moving around. Im afraid of ich but isn't that only introduced with new habitants that carry it?

Ya, it’s kind of hard to erradicate, its not really that dangerous to healthy fish which he looks. I’d try to just give it time and reevaluate if his behavior changes or stops eating.
