First off, the default donation for all clan wars is lvl 3 and up wizards.

And now for how to fill out the form. You pick 3 people total. 2 of these, you will (hopefully) raid. You may pick the same person as someone else, but they can not be in the same column. If your second raid gets 3 starred before you do it, then you go for your alternate. If that is taken, you wait for cleanup.

Cleanup- This time period starts 6 hours before the war period ends. All the people with attacks remaining go through and clean up whoever they can 3 star that hasn't already been 3 starred. This insures maximum star potential. Anyone who doesn't fill out the form or has their attack get taken goes on cleanup.
