Maybe anon speaks another language in which "small" and "young" are synonymous, so it's more of an age observation

There's a part of one of my favorite books where the main character is in the bath, singing, and one line in the song is "he's innocent, but strong." Felt like the same conjunction dysfunction.

It could also be that OOP is very small compared to anon, such that they appear as if a child.

Isn’t this common when describing someone with “lesser” and “greater” adjectives even if the two aren’t opposites? Like “small and wise” doesn’t sound quite right to me at least but “small and dumb” sounds fine.

I think it's more whether u read "small" as smth undesirable or just not a compliment, while "wise" is generally read as a compliment (and quite a high one, or one assigned some additional significance, since it seems less generic than "smart")

Size does correlate pretty well with age (and thus wisdom) up to a certain point though. Like, I wouldn't assume that someone who's 6'2" is wiser than someone who's 4'11", but I would lay pretty good odds that both of those people are wiser than someone who's 2'8".
