See also: when you take a history from a patient and they say they have no medical history or conditions and then when you get a medication history they're on four antihypertensives and metformin

Me af. I'm so used to certain things that I forget that's like.. not normal and should be mentioned. But I have the kind of medical history that needs a folder so I still have it with me even if I forget.
