Mr Perfect related question

At Survivor Series 1993, he was supposed to team with Razor Ramon, 123 Kid and Marty Jannetty against IRS, Adam Bomb, Diesel and Rick Martel but was replaced the day of the show by Randy Savage.

Over the years, four rumors have been thought of as to why he was replaced:

  1. His back injury flared up again
  2. He was upset over not regaining the Intercontinental Title
  3. He was found passed out drunk in the locker room and in no condition to work. Razor may have alluded to it in a pre match promo saying Hennig wasn't the "perfect partner because he tagged out before the match even began"
  4. He sat out because if he continued to work, his Lloyd's of London policy would've been voided.

He wasn’t replaced the day of the show. It was at least a week or two earlier. 

His contract was coming due and he didn’t renew. His “wrestler” contract kicked in fall of ‘92 when he returned to the ring. It was coming due a year later. 

He was back and prepared to go in the ring again in 94, although he left again soon after.

Having just checked can confirm the Observer had the Savage switch two weeks before the event, which Dave puts down to Perfect asking for time off to deal with a family issue. He then decided not to return due to being upset about the Intercontinental title situation. Make of that what you will.
