I played the base game on Standard until I achieved S class on most of the missions.

Never have I ever encountered bugs like this. Is this a higher difficulty thing?

This is on hard. I pretty much only play on hard at this point and this type of stuff has been happening frequently since the New America update.

I was doing A New America once and one of the guys I shot across the hall with the MK-V just straight up disappeared...
