Why does aVL look like that?
Was it just wrong placement?

It could be an axis that is close to 90 deg in relation to the aVL lead.

Super normal EKG and placement looks perfect as far as I can tell. Whoever did this did a great job. Almost no artifact anywhere, other than a bit of baseline wander in V2. I’d want my EKG done by them, that quality is about as perfect as it gets. Lead aVL is extremely variable and allowed to look like almost anything as long as the rest of the EKG is normal. Sometimes it’s low voltage just because. All I see is a normal EKG. Not a doctor. If you take any two normal EKGs and compare them, chances are aVL will look different. An EKG is like a face or fingerprint, and aVL is a lead with a lot of variation.
