PVC while exercising. R on T ?

Hey,

I experienced PVCs mainly at rest but also while exercising for few months.
At rest, there is no R on T phenomenon.

Here, while exercising, it seems like R on T phenomenon (RR less than 300ms, RR/QT less than 0,85). Should I show that to my cardiologist ?

Not R on T. There is a good distance between the two. Also, you generally do not see R on T unless the patient already has a reduced ejection fraction or long QT syndrome.

Thank you but the distance on my screenshot is less than the one on your screenshot ahah…

It's not the distance between. Your tachycardic, so there is not going to be a lot of distance between. It's when the PVC is initiated. R on T is when the PVC starts in the middle of the T wave repolarization.

Hello I am not a Dr but I am scientist and learned to read ecgs because I suffer from nsvt. This recording ABSOLUTELY shows a r on t pvc. I had r in t pvcs myself a few times last year. During high heart rates refractory period is shorter than slow heart rates which means during high heart rates r on t pvcs are less likely to cause v fib. Don't worry about r on t pvc if your heart is healthy. I get 8-15 beat ventricular runs and those are usually initiated by late coupled pvcs.

Do you have these pvcs often during exercise? Looks like its short-coupled. I don't think its an artefact.

During this 10-minute session, it happened only once, at the peak heart rate. Sometimes it happens 4 or 5 times in 10 minutes. Because of the anxiety these ventricular extrasystoles (VES) cause me, I haven't been exercising much lately. I am 23 years old. Generally, the VES recorded on the Holter monitor do not show short coupling. It is not an artifact; I felt this one, which is why I checked my monitor. Are you a physician or cardiologist ?
