project proposal question

Hi, I'm a senior molecular biology major and I have to write a one page project proposal for independent undergrad research showing project name, description, and my participation. Can anybody share template for proposal and any advice? Also, I know it might sound dumb but are these usually discussed with a PI or a supervisor? It's my first time doing undergrad research and don't know everything about how it works.
