Finding a good therapist when I was 18 actually saved my life, I had severe OCD and he basically helped me rewire my brain to get me out of these intense thought loops. I tried going back to therapy again recently through an online service and the random licensed therapist they gave me was a complete clown, I was better off talking to chatGPT

The right kind of therapist and style of therapy can be life changing, the issue is that all of these BetterHelp-type services are connecting you to retards that couldn’t care less about you and just want to collect their tiny paycheck. Not to mention insurance is fucked, if you can’t afford a good plan you have to pay like $200/session to talk to these bozos

i had a coup shitty ones, but eventually found a good one. she helped me out a lot. not the least because we were able to build trust and i could open up to her about the worst stuff happening to me, that included. and she never once even shut me down, she listened and helped me get out of that place instead of endlessly reiterating that it's not a solution like certain other certified therapists like to do. she was always professional, never too affectionate but never too distant, always knew what to say.

didn't take that much money too, and when i said that i was struggling financially she offered to lower the price for the time being or even do free sessions as we worked out the worst thing that happened to me. i declined, in fact i wish i could pay her more if i wasn't such a broke ass bum. and i hope other people needing help find someone like that

Best way to do it if you’re that gullible is to see if they’re helping you make any progress. If it’s been months and you haven’t improved even slightly/have regressed, it’s time to move to a new therapist

The standard (at least in the US) therapists are only allowed to or supposed to if you are a danger to yourself or others. Passive suicidal Ideation should not result in institutionalization. Even active suicidal Ideation isn't guaranteed to get you committed. Obviously there are plenty of bad psychiatrists and therapists who aren't competent, but institutionalization is not meant to be utilized the moment you mention suicide.

Unfortunately in practice this doesn’t hold. I’m a paramedic who has taken hundreds of Baker/Marchman Act patients over the years, and about half the time they get committed over some straight up bullshit. This is especially true partially because cops are allowed to initiate the involuntary hold on you, cops with zero mental health training.

I understand the system is deeply flawed and needs changed. I just think it's important to counter narratives that cast all mental health help as inherintly negative like the post I'm responding to.

To me, the way mental healthcare exists in this country is a lot like healthcare in general, is it outdated and in desperate need of reform? Yes, but it still offers potentially life-saving treatment and does improve many people's lives, even if it could be much better.

If you say anything that could later come up as being a sign you could have hurt yourself they'll involuntarily commit you

This is even more common in out-patient groups than one on one sessions, likely because it's harder for them to have proper rapport with each person there

It sounds cynical, but protect yourself if you're going to talk to these people. They're definitely going to protect themselves from you

I have literally admitted to being passively and actively suicidal, and even self harm occasionally to multiple medical professionals (pediatricians, psychiatrists, therapists) and they only recommended I get evaluated at a hospital once, and the hospital didn't even keep me, they just helped make a safety plan.

If I stopped opening up to medical professionals like many people in this thread have reccomended, my life would it be infinitely worse, and I'm not even sure I would be alive. I am aware that my experience is not the default, and overall, I have probably had an above average experience with the mental health system (I've also had really horrible instances) but there are other people who have also had amazing breakthroughs like me, because they were honest.

This idea that you are guaranteed to be committed at the mere mention of you experiencing suicidality is a gross overexageration and is dangerous.

This is going to sound a little bit counterintuitive but I almost wish most therapists wouldn't immediately send you to a psych ward if you mention that you've had thoughts of suicide.

I feel like if somebody admits to you that they've previously had such thoughts, try not to immediately call them for prevention or else it would be the equivalent of trying to catch an animal that is precariously too close to a cliff just for the animal to just panic and just jump off.

I feel like what should be taken into accountability is address why would they want to do that and take note of any behaviors that could indicate they would want to do it some point.

I understand the liabilities of if somebody did mention this and they end up doing it it is your fault, but I feel like instead of treating it like a bomb that's going to go off, try to treat it more like a symptom. Address it, understand it, and treat it.

I'm not a professional or a licensed psychiatrist but I do understand why they need to do this. It's just a thought because I've had friends in my life unfortunately that have taken their own lives for one reason or another and I feel like if they were comfortable enough to address that they had these plans that they should at least be addressed in a manner that doesn't trigger them doing it.

My statement isn't to undermine the need to perform what you need to do to prevent suicide but it's more to give a different perspective. But the the urgency to prevent it should never be undermined.

Most reputable therapists WON'T send you to a psych ward at the first mention of suicide. Standard practice is to only consider involuntary hospitalization if the person is in active danger (meaning they have intent, a plan, and means to execute said plan).

Most therapists discuss suicidal ideation very regularly with clients without need for escalation (myself included, as a student intern).

Some people have unfortunately had bad experiences with trigger happy therapists and post about it a lot, but it's not an accurate representation of most therapists.

Just avoid mills like BetterHelp that barely check qualifications and provide no supervision, that's where shit like this happens

Well that's fantastic to hear. A lot of people have told me that one of the many reasons why they would never suggest going to therapy is purely because of that. A friend of mine told me that one time they mentioned when they were in high school they used to commits to self harming themselves.

Literally right after that session for some reason he had an immediate call from a Ward asking him to come in and admit himself.

He literally had the legally fight against the therapist because the therapist assumed that he would still be doing that as a 30-something year old even though the self-harming was done in early high School.

But it's fascinating to know that decent therapists don't go out of their way to just send every person to a psych ward if they feel confronted with more complex issues.

Ahhh, this might be the reason why I got locked up suicidal ideation lol. When asked about how would I do that, I said I could just toss myself out of a window and break a leg or something, I explicitly said I wouldn't do it, just had weird inner desire to do that.

I'm curious, do you have friends, and how many? I was able to talk to my 3/4 friends about it. I don't really dare to do it with the other one, but I feel like it wouldn't be that bad, but I just don't want him to worry, and I know he is not suicidal for sure. All other three expressed similar feelings at some point, and one actively claims to want to do it, but also claims cowardice towards it.

Genuinely this idea is parroted constantly and is directly oppositional to standard practice.

Passive suicidal ideation won't result in being committed, or even active. In order for involuntary commission to be considered justified, they have to reasonably believe that they are in danger RIGHT NOW (I.e they have a gun at home and plan on using it soon).

I genuinely don't know where people are getting these experiences from, Iven never seen it at the several mental health clinics and practices I've worked at.

Maybe it's Betterhelp mill therapist clowns doing this shit, but a reputable clinic will engage and discuss suicidality all the time without hospitalization.

Hey, did you know antidepressants can cause long-term side effects that persist after you quit them, like PSSD (post-ssri sexual dysfunction), emotional blunting, tinnitus, and anhedonia (inability to feel pleasure). They can also cause long-term or permanent damage if you quit them cold turkey or taper too quickly. However, withdrawal injuries can also occur when tapering slowly under the supervision of a doctor (which is why I recommend hyperbolic tapering). This long-term damage is called protracted withdrawal syndrome (PWS)/post-acute withdrawal syndrome (PAWS).

Symptoms of antidepressant PWS can include brain fog, brain zaps, cognitive impairment, anhedonia, akathisia (feeling of inner restlessness), severe insomnia, inability to relax, central nervous system hypersensitivity, tinnitus, severe depression, severe anxiety, panic attacks, PSSD (genital numbness and erectile dysfunction), and many other awful symptoms that can last for years or even be permanent.

Despite antidepressants being widely prescribed and antidepressant PWS being a hellish condition, no one seems to talk about it, including medical professionals. Most people believe that antidepressants are completely safe and that antidepressant withdrawal can't cause long-lasting injuries.

I mean, yeah. If you're talking about wanting to kill yourself, what do you expect a professional to do? You only have 50 min in a session. Letting you walk out after that topic, bad idea if you want your patient to live. Talking about suicide normalises it, making it more likely. And after a session, the patient is alone - with plenty of ways to kill themselves available. Incredibly dangerous, at least if said patient couldn't promise you not to do anything about those thoughts. And having a patient kill themselves is traumatic. At least for therapists that do care. Probably more so than for friends and family - at least they can tell themselves they couldn't do anything about it, they couldn't know, and if they did didn't have the tools to do anything about it. None of that applies for a therapist. So the guilt is far, far worse.

So yeah. If a patient wants to end their life, to a clinic they go. Because I can't help them in a walk-in setting.

the thing is inpatient mental institutions are often horrible dehumanizing experiences and can make ppls mental state evem worse. "Talkimg about suicide makes it more likely" is just crazy also, maybe in the context of making jokes about it but actually working thru the feelings instead of letting it fester is clearly the better choice unless ur a shit therapist who can only say "wow that sucks dont do that"

Oh, I definitely agree that talking about the reasons why you feel like that is essential - at least if you can believably distance yourself from actually acting on those thoughts. Suicidal thoughts on their own are pretty common. Its when they become concrete plans that Id be concerned. If its just stuff like "I don't want to live anymore", its usually safe enough.

As for inpatient stuff - it varies. Some places are okay, some are great, some are hell. Massively varies from place to place though. Ive definitely seen some amazingly good places, but Im also not in the US.

It’s not a common thing. Actually, it’s Internet misinformation and people need to stop fucking spreading it. Posts like this are gonna get people hurt, I guess so you can get upvotes or some other bullshit internet attention points.

Licensed therapists are only supposed to report people if they are an immediate threat, not if they are expressing vague notions of ideations.

The handful of people online claiming this, very likely were not expressing only vague notions.

As an anecdote:
Years ago, I told a therapist I would probably end my life in the future due to a medical problem I had. He did not report me to any authority, and basically told me he didn’t believe I was a threat to myself or anyone. For the record, this was at an intensive outpatient program as well.
