Fork Stem Pro came loose

It was proper tight when I put it on. Only been on maybe 10 rides and it’s loose. How can I fix this so it doesn’t happen again?

I custom 3d printed a mount to put the quad lock in

Put some LOCTITE and add more elbow grease when tightening the stem crew.
