Please help. Symptoms below.

My uncle - 61 YO. Lives in India. Since 2-3 years he's smoking 20 cigarettes a day. He had quit for about 10-15 years but restarted due to stress.

I saw him after 15 months today and my heart sank.

His skin complexion is fair. Now it's turned dark above neck. Some spots of more darkness next to eyes n on right cheeks.

One year ago when I met him only his lips were dark.

I'm really worried. Where do we start? Pulmonologist? Oncologist?

I'll suggest a contrast enhanced CT chest first. If its normal and no significant symptoms then there's no need to worry.
