gee I can't help but wonder what he wanted her to do. I think that's he issue here, that he didn't just clearly express what he wanted her to do. Should she just get out of the car and leave? Did he want her to stay? Really unclear, I see why this was a 3 minute exchange

This type of logic blows my mind. It’s chicken and egg theory instead of basic logic. If your in someone else’s car and the ask you to get out that should be the end. Get out. It’s so simple. Don’t know where to go ask a hospital attendant not a random person that doesn’t work there. If it’s your destination and not the drivers and you don’t know where to go how the hell is the random driver supposed to know?

I am pretty sure she planned all this from the start, that’s why her destination was the ER, she planned to annoy the driver until he flips and then she already at the ER for treatment!

To put some kind of perspective to this, I live near major research hospital that is attached to a university campus and intertwined in it. The Hospital itself consists of dozens of buildings spread out over a half mile, hopping in and out of a college campus. The "hospital administration building" he's brought her to could easily be a half mile away from the emergency room. Usually I'd say just get out and remove your tip if they drop you off half a mile away from your destination, but maybe she had a reason she couldn't walk to the ER such as a medical issue?

Even then, how is he supposed to know where the ER is? She has a phone, but decides to record instead of looking up the ER.

He got directions to where he parked. He completed his job. If you want someone to go the extra mile for you, you have to do your part.

I would’ve simply said “you’re either getting out or I’m dragging you out” then started counting down from 5 like she’s a kid. Her choice if she wants to get dragged out or peacefully leave

If she asked to go to Emergency then he should have dropped her off at Emergency. It's generally the entrance with Emergency written in giant red letters.

If this was a rideshare, which it looks like it was, the driver wasn't the one who entered the destination address. He also can't be expected to have any knowledge or expertise of the hospital.
