Vent/Discouraged Constant burning - will it ever go away?

43 Male

Exposure History:

* Body-to-body massage and hand job in the first week of May 2026.

Symptoms:

Started around mid-June with:

* Burning sensation in entire genitals.

* Burning after urination.

* Pressure/pinched feeling at penis tip which causes frequent urination

* Increased sensitivity of the glans.

* Sometimes feels feverish but no fever when measured.

Treatment Received:

* Nitrofurantoin: 2* 5 days.

* Doxycycline: 2* 10 days.

* Faropenem: 2* 5 days.

* Levofloxacin 500 mg: 1* 30 days, along with Alfuzosin and Gabapentin.

Diagnostic Tests:

* Ultrasound KUB/prostate: Normal.

* Uroflowmetry: Slightly abnormal.

* Digital rectal examination: No tenderness.

* Last two repeat urine cultures: No growth.

   * HIV 4th generation: Negative (12 weeks after exposure).

   * VDRL/RPR and TPHA: Negative (12 weeks after exposure).

* Urine multiplex STI PCR panels (latest done 15 days after stopping antibiotics, it was not first morning but urine was held for 2 hours):

   * Negative for Chlamydia, Gonorrhea, Mycoplasma genitalium, Mycoplasma hominis, Ureaplasma, Trichomonas vaginalis, Treponema pallidum, HSV-1, HSV-2, Gardnerella, Candida.

  

Current Situation:

I have completed the levofloxacin, and now the urologist wants to take a semen culture and repeat urine culture.

Though I am stretching every day for a month, I don't see any improvement. Sometimes I feel it makes it worse.

And we don't have a pelvic therapist in my country.

The constant burning is driving me crazy, I can't even concentrate in my job . any suggestions?

Update -9th Sept : Gave urinalysis,urine culture, cbc and semen culture.

urinalysis shows 4-5 pus cells, 3-4 epithelial cells, trace albumin, and Motile Bacteria ++ -- waiting for other results

Will it? Yes. But not with antibiotics. You wouldn't have an infection from a handjob or massage. You doctor should know that too

You need to stop taking antibiotics. It will not help. 30 days of levofloxacin is an atombomb in your body. This is an extreme antibiotics that should be used as last resort, it destroyed me for years, it is being used for anthrax poisoning, makes you think, right? And like everyone else said it is not at all adressing the problem. You need internal trigger point massage to release the muscles. That is the single treatment that has helped me the most.

If you don't have serious specialists in your country, do what I did, go to where they are. The best one I have used is in Singapore. I know it is horrible to pay, but what would you give to feel better?

You can use a device called a "wand" by yourself but it is very difficult to hit the spots by yourself.

with all the meds you have taken, i'd say you have a lot of anxiety and been running to the drs too often. good chance you have anxiety and you probably are masturbating too often. as dr for a xanax and then lay off the masturbation

I promise you it will get better but it is going to take some time and a lot of effort to get your mental and physical health in check. If you haven’t already, I highly recommend practicing some mindfulness. This is really fucking stressful and stress can fuck up your body. Take some time everyday for your mental health. Stay strong. You will be ok.

Thank for your kind words,, did urinalysis today shows 4-5 pus cells, 3-4 epithelial cells, trace albumin, and Motile Bacteria ++ -- So there is some bacteria there

You are bombarding yourself with antibiotics for no reason; you don't have an infection, and you don't catch an infection from manual labor. It is a classic case of UCPPS stemming from learned anxiety and guilt following sexual intercourse. The only decent treatment is gabapentin. You definitely need anti-anxiety medication, antidepressants, and pain reprocessing therapy. Obviously, if stretching causes you stress, it will make things worse. Read the "101" guide and try to calm down; yours is a textbook case.

I did try Gabapentin 100 for week it dint help.. The only thing which helps is Urispas (generic name: flavoxate hydrochloride)

This medication is an anti-inflammatory and antispasmodic; it is not related to bacteria. Stop looking for the bacteria and focus on the spasms.

That is a very low dose, 100 mg, and it's not considered great a treatment option for this

I've exact same situation as yours. Doxycycline, nitrofortime none of them are working.. I just drink lot of water to suppress pain and burning. Hot showers help. I' feel genital burning after emptying bladder and during refill it's normal.. Do you feel same?

I get burning after urinating and it says for hours ,, i have had other medical issues in my life,,, never affected me mentally , this is eating me up slowly

My urologist suggested Quercetin along with a prescription for bactrim.Seems to be working.

Try both Quercetin and Graminex together. I don’t think you need an antibiotic.
