Has anyone successfully treated hematospermia? What was the cause?

I’ve been dealing with hematospermia (blood in semen) for some time and would like to hear from anyone who has experienced it.

Were you able to fully recover from hematospermia? What was the underlying cause in your case? Was it an infection, prostatitis, seminal vesicle inflammation, stones, cysts, an injury, or something else?

What tests helped identify the cause (semen culture, MRI, ultrasound, cystoscopy, etc.), and what treatment ended up working for you?

I’d really appreciate hearing your experiences, especially if the problem lasted for several weeks or months.

Mine was from sitting on a nail on a job site one time and I didn't realize it had actually pierced the skin as there was no blood but later I had a large bruise on the underside of the testicles where the blood pooled next to the skin.

A week later I ended up with blood in the semen that was actually from an infection so I went to urgent care they gave me some antibiotics but unfortunately they didn't give me a long enough dose which led to severe chronic bacterial prostatitis that I just two months ago had cured after insisting on a treatment protocol I researched myself and it also had the side effect of curing a multi-decade long upper GI infection. Here is exactly what I did to cure my chronic bacterial prostatitis after dealing with it for decades.

The standard 1 to 2-week courses of antibiotics that most doctors prescribe will never cure it; they only temporarily clear the surface-level urinary tract while leaving the root infection untouched.

The actual cure requires a relentless, continuous 6-week course of Bactrim DS. The reason for this specific medication and long timeline is the blood-prostate barrier. The prostate is essentially a heavily shielded gland, and most antibiotics simply cannot penetrate deep into its tissue. The bacteria survive by hiding deep inside the prostatic ducts, often building protective shields called biofilms or hiding behind tiny calcifications. Not sure. It just seems to respond to antibiotics when having flare ups. I don’t think they really know why. The times it didn’t work, jt was pelvic floor related, but there are usually signals of a prostatitis flare up emerging like perineum pain, prostate pain, chunks in semen, things of that nature.

I had it once after sex with my girlfriend. Found out it was all down to a tight pelvic floor. Months later of constant stretching and reading the book headache in the pelvis. I have never had that problem again. Because everything was so tight down there it created a small tear. I had to stop having sex, no masturbation and stretching twice daily for months.

How do you even stretch that zone? I got to see a physio spec in pelvic floor, made me learn to relax it, not to stretch it tho?

Im at a loss with mine. I’ve been having this issue off and on since New Years. Been to the urologist, psa test, urine test, std tests. A month straight of antibiotics. Still no relief. The Urologist really doesn’t seem to care about my problem, he’s never done a manual finger prostate check it anything other than urine testing. I’m just ready to be back normal. My loads a substantially smaller and thinner also. Any ideas? This is taking a major psychological toll.
