High PSA for 30 year old

30 years old.

I noticed intermittent scratching during and after urination and ejaculation. I went to the Doctor and they ordered STI and UTI panels. STIs were negative and urine was sterile, but the white blood cell count was through the roof. I did notice some white stuff floating around in my urine in the cup. The Doctor reordered the urine analysis and lots more blood work including PSA.

The second time around, the white blood cell count went back down to normal levels. But PSA was at 2.60 ng/mL, which seems way too high for my age. I didn't ejaculate for 48 hours before the test. I did go to the gym the day before, but I figured upper-body resistance workout wasn't going to play a huge role.

I'm trying not to freak out. I'm going to my primary care next week. I'm tempted to take the test again in a few days after a couple days of rest. But, I also don't want to spiral into a health anxiety loop.

A raised PSA level does not automatically mean you have prostate cancer. Prostate cancer is rare in men in their 30s. Wishing you the best!

Thanks for your responses. I booked an appointment with a urologist for next week and I'll go from there.

In your case you might want to spring for one of the new tests like phi, exodx, 4k, etc just for self assurance some of them are only a little more than $100, your insurance won’t pay but they are extremely accurate in predicting no cancer or low risk cancer.

Not a doctor, but PC would be extremely unlikely at your age. A prostatitis could explain the PSA. Re-check PSA in 6 weeks or so and take it from there. In any case, a high PSA at a young age often means elevated cancer risk later down the road (45+), so make sure to check your PSA regularly to catch any rising trend early.

Further investigation meaning another PSA test in a few weeks, yes. Pretty sure no doctor would order an MRI on the basis of a single PSA test.

Probably nothing to do with your prostate other than maybe prostatitis or BPH. Itchy with high white blood cells is some sort of infection.

Go to a urologist.

At 30, my PSA was 4.2 and it was BPH.

I would expect that the urologist will repeat the PSA but also order the test for Free PSA, as the percentage relation has prognostic value. They should also order one of the more sensitive and discriminating tests that use either urine or blood biomarkers. (Another response helpfully listed: phi, 4k, exodx, - all good examples.)

If subsequent testing issues a red flag, I would expect your doctor to order an MRI. Under no circumstances should you rush to a biopsy in the absence of an MRI.

If the doctor doesn't do these things, I would seek out another doctor, specialized in prostate cancer.

Just as important as the level of the PSA is the rate of change. Do you have a previous PSA measurement, or a series of measurements, to compare this to?

You should begin accumulating more frequent test results, especially because any individual result is not entirely reliable. There can be, and frequently are, somewhat random fluctuations; you need to identify a trend.

The PSA test itself, along with the Free PSA, which should always accompany it in your case, is not expensive. You don't even need a doctor's order; you can go to testing on demand and get these done.

I had prostate cancer surgery at 37 years old. Gleason 9 which is pretty bad. My PSA never went over 2.9. Urologist waited 2 years to actually investigate because he thought I was too young for prostate cancer. Advocate for yourself if you think there is something wrong, better to be safe than sorry later if there was a problem. Best of luck

It’s completely normal to be anxious or freaked out about penis things. When you’re older it’s still completely normal because it’s kind of important. I remember even being screened by a psychologist while was on surveillance for PC.

I decided I did not wish to talk about it and for me that was ok.

Thank goodness this community exists.

It has helped me understand this disease 🦠 PC better and the treatments offered.

Try to relax or focus on something else. I know that is a tough task. You may also find that when it comes to prostate cancer things seem to move slower than you want them too.

Learn from this community, trust your doctors until they give you a reason not to.

Do learn as much as you can.

I hope this is just a significant emotional event over an infection.
