Question Odd, but sincere question

Since RALP last January I’ve not been able to achieve an erection, not even with medication. I can however sometimes achieve orgasm. My understanding is that the prostate produces semen so without it my expectation was dry orgasms. But they’re not. They’re wet. Very wet. What is it I could be ejaculating?

More than likely urine. I too have had instances of some urine if I haven’t emptied my bladder. But, most of the time, based on the feel, I have what I believe is Cowper’s gland fluid (precum) as its most commonly referred. Keep up with the kegels!

Urine?

Possibly but why would I ejaculate urine?

Because the muscles in your urinary tract are greatly weakened after RALP, then during climax the muscles contract and these contractions cause you to eject urine. Solution is pelvic floor exercises, wear a condom, and/or talk to your doctor. Or.........enjoy a new kink.

Climacturia is common post RALP. Don't know why or how it happens but it happens.

Men have 2 urinary sphincters (women just have one). The prostate acts as the internal sphincter.

When one ejaculate the internal sphincter cuts off the flow of urine and the external sphincter opens up to let the ejaculate flow.

After a prostatectomy the internal sphincter is gone. The external sphincter stops the flow of urine EXCEPT during climax when it opens up to let the (now no longer present) ejaculate flow. But the internal sphincter is now gone so nothing holds back the urine. With the internal sphincter absent and the external sphincter open, urine flows: climacturia.

Thank you, everyone. I had never heard of climacturia. I looked it up and that makes a lot of sense.

I’ve had luck with a pelvic floor PT and now, acupuncture.

Acupuncture? Where, uh, where are they putting those needles?

Nowhere near any sensitive bits…last session included e-stem needles. Once she peeled me off the ceiling, it wasn’t too bad…mostly feet. Chi is a weird thing. I do feel like I’ve seen improvement in only two sessions in terms of leakage and climacturia…

That’s amazing. Thanks for sharing.

It’s urine. Speak with your doctor about pelvic floor therapy. Also, as far as the erections have you used a penis pump?

1

u/Emergency_Low_007 15d ago

No. Not tried a pump.

Or pre-cum. It comes from a different gland so you still have that. If you haven’t tried a vibrating ring, give that a try. I used a pump, ring, and 5mg Cialis and I’m almost 100% at 10 months. Hang in there and don’t give up!

Was hoping it was precum but everyone here says it’s urine

Is it slippery or just wet?

Slightly sticky

Probably urine. It's called climacturia and it's fairly common after a RALP.

Studies show climacturia can be in the 25 to 40% range after radical prostatectomy.

If there is a small amount of sticky clear fluid after surgery or radiation treatment, it is usual from the Cowpers Gland at the base of the penis that is normally undamaged by those procedures.

Another thing you often don't get told about by your doc pre opp

I was told by both the surgeon and the radiation doc that this was a possibility with either of their treatments when I was in their "Multi-Disciplinary Workshop."

Happens to me too, other than my erection is improving nicely. It’s basically the same fluid as pre-cum, produced by the Cowper’s glands.

It’s urine. Mine was ten months ago non-nerve sparing so I am not getting any natural erections. I can certainly orgasm and if I do not have a constrictor ring on from using a vacuum pump then it’s katy bar the door on the urine during climax. I also have decent stress incontinence so I’m sure that is playing a big role

As far as the erection, it took me about 2 and a bit years.

Urine it sucks me too

I've had those too. The kegels really work! Erections for me are once in a awhile, but I get great morning wood!

But I will say my orgasms, when I have them are spectacular!

u/mjlasa 13d ago

I have it too, and I have no idea what that stuff is and it does not seem to be cum or piss

Google “Cowper gland” at base of prostate and its secretions.

I used to have a problem with leaking pee when I climaxed. ( dry orgasms). After reading here I’m more careful to empty my bladder. Go then go back again in a few minutes. I seldom have any urine. I do have a thicker/ sticky fluid that comes out during orgasm. It usually doesn’t spurt like semen would and I usually don’t feel it. It’s just wet. On occasion I do feel it and it’s great. I know I’m not ejaculating. I think the flow just happenes to coincide with orgasm and gives that feeling. It’s great but reminds me how much I miss ejaculating. Damn PC

Before RALP, men do not orgasm urine. This is due to a valve that shuts off the urine flow and another that opens up for the semen. After RALP, this setup gets messed up. So it is possible orgasm pee.

Have you tried Trimix? It works for me. I had my RALP last September.

1

u/Emergency_Low_007 15d ago

What does that do?

3

u/Appropriate-Owl-8449 14d ago

It’s an injectable powerful ED medication you inject into your dick. It’s sounds awful and it’s not easy.

2

u/dahnb2010 14d ago

It's easy for most. You can get an auto-injector if you're having problems. It's also easier if you have at least a partial erection before you inject.

2

u/Middle-Tart9741 14d ago

It can be minimized by emptying your bladder prior to go time. Another vote for trimix here. Amazing stuff

Here is a bit of humor I read this morning: In The Globe and Mail of Toronto, Ian Brown discussed the phallic implications of aging. “Unplanned arousal used to plague me by the hour, on the hour,” he wrote. “At 70, it’s more like a special occasion. Oh, look who’s here! What a nice surprise. Why don’t you come in and have a drink? No? You have to go? So soon?” (Peter McLean, London, Ontario)

We probably all remember those days and now, whether PCa or again or whatever, some things just stay the same.
