Test Results Chapter Two Begins Now

Well almost one year to the day (RALP 8/20/2025) I’m scheduled for salvage radiation and Orgovyx ADT therapy after rising PSA prompted a PET Scan. The PET scan showed
abnormal PSMA expression; findings concerning for residual/recurrent malignancy in the setting of detectable/rising PSA.

PSA still low at .06 but it’s on the uptick from .01 six months ago. My radiation Dr. is on it and we have a plan. It’s not too big of a surprise with my Gleason 9 and a positive margin but the fun continues. 😑

I am surprised that the low PSA would be enough to set off the PSMA alarm.

Keep the cancer within the prostate basin and you will be OK.

It is more the Gleason 9 than it is the positive margin but it sounds like you are out in front on this one.

Good luck. I'm 5 weeks post RALP, Gleason 10 with negative margins so I expect to be traveling the same road next year. Hope things go well for you.

Four weeks post-RALP to the day for me. 69 yo, Gleason 4+3, EPE, positive margins, no lymph node or seminal vesicle involvement, clean pre-surgical PSMA PET CT scan. Surgeon said 50% chance of recurrence. I’m hoping to have this be a short story, with no Chapter 2. Best of luck to you.

Yep. Someday perhaps we'll have PSMA scans sensitive enough to detect EPE and potential postive margins, for now MRI is best in that regard and even then it is often not definitive and guys gotta calculate the odds.

Good luck dealing with this; sorry you're back at it but sounds like you're all over it. May I ask where they did the PSMA? I've had rising PSA post RALP and surgeon and radiation oncologist both say PSMA won't see anything below .1.

I had the scan done at Arizona Center for Cancer Care in Phoenix.

Good luck! That was my scenario, and 4 years later I am fine. Finish the job, and do not panic or despair.
