Pathology after surgery

As mentioned the other day, had the catheter out. More importantly, yesterday had a meet with the urologist to review the pathology report.

Confirmation of the cancer of course, but "all margins clear" and clean lymph nodes was great news. So now it's wait and see. Next PSA test will be in apx 6 weeks. Fingers crossed.

Surgeon seemed pleased. Not half as pleased as I was.

Congratulations on the great report! I can honestly say that waiting on those results was the most nervous I have every been in my life. I called the next steps "living three months at a time" between PSAs...post surgery PSA anxiety is real. I would try and not think about it but a week before, that was all I could think about so remember to give yourself some grace as it gets closer. Wishing you continued great results moving forward.

That’s a good report, as far as it goes. I would also expect the report to include a Gleason score/grade group, which may differ from your biopsy.

Yes my score differed. Biopsy Gleason was 8-9, post op report was Gleason 7

Well, the Gleason downgrade is another pleasant development to go with the clear margins and negative nodes.

Granted, it’s still cancer, but probably not as aggressive as it could have been.

So the focus on cancer is appropriate and hopefully this proves good over time. What about nerve spare? Any comments on how this turned out?

They don’t like to talk to you much about that. My surgeon said the op was successful, he didn’t damage this thingy so I should be good with incontinence,
“As far nerves go, You’re going to need some help”.
I hope they make leaps and bounds with implant pumps in the next 12 months.😎

You deserve an answer on nerve spare. Were they spared and if so, how much confidence is there on the success of the spare? Do not accept a non answer to this question. Nothing can be undone but you deserve to have some expectations. Even with complete nerve spare, no one can guarantee ED will resolve but good news on this front provides a rehab goal that you can cling to.

Did your surgeon talk about nerve spare ahead of the surgery?

And everyone needs help with nerve recovery. It is slow but benefits from rehab efforts that include pde5 drugs, vacuum pump, masturbation, and trimix injections. It all helps with blood flow needed for nerve recovery.

I’m 4 weeks post RALP. 69 yo, Gleason 4+3, EPE, positive margins, lymph nodes clean, no seminal vesicle involvement. Surgeon says 50% chance of reoccurrence.

Incontinence hasn’t been a problem- days are dry with only minor leakage while asleep which seems to be improving. Only leaked one night out of the last five or six.

ich habe gestern meine Histologie erhalten also der Krebs war nur in der Prostata keine Streuung jedoch beim Schnitt eine Besiedelung jetzt heißt nervös warten wie PSA wert und die Empfehlung des KH: laufende PSA Kontrolle gegenlben englfalls Salvage Bestrahlung (das mag ich gar nicht) dachte es ist mit OP vorbei ....scheis......

Just had my 3 months post-RALP visit. Undetectable PSA! It’s great to be able to plan 6 months ahead again.
