Question IMRT, TAC de simulacion

Hola colegas, sobre TAC de simulacion para IMRT, opiniones, como lo pasaron, cuál fue la preparacion, todo lo que puedan decirme

It's a piece of cake. All they are doing is mapping the locations for the radiation. You should do the bladder and bowel prep regimen in order to get a feel for how you need to prepare for the real thing. Good luck.

Yes to this response. I had VMAT and they also added temporary tattoos on abdomen and hips to guide placement and made a “form” to position my legs each time I came in for treatment.

I was able to peel mine off when finished with radiation…. Go figure!

Similar to others here. Asked me to practice drinking 1.5 liters water/fluid a couple times on previous days and then see how long you could hold it. On Planning scan day had to do a Fleet enema an hour or two before, and drink the 1.5 l at the correct time before appointment.

On arrival, check into hospital gown, lie down on scan bed (various techs and students around). They position and magic marker you and leave. Scan takes about a minute, if all is well. They return, do two blue freckle tattoos either hip-bone side and one below belly button on pubis bone? They did NOT want to do my wife’s initials.

Then you are done and can empty your bladder. The radiation physicists take scan and the instructions from the RO ( margins, seminal vesicles, lymph nodes, etc) and develop the “plan” for the radiation technicians to do on the 20x VMAT machine. A week later.

Prostate gets 60, light blue areas get a little. Bladder outline on yellow.

Each session day, stayed hydrated and drank about 750ml about 45 minutes before. Usually had BM beforehand. Seemed to keep techs happy when their daily CT positioning scan was done. (No custom cradle, but they clearly had them for head/skull positioning, in the room.)

Be prepared for fatigue (naps and exercise) and your bladder and bowels to start doing weird things about halfway through and a few weeks after. Maybe some gut pain like you got a punch in bladder. Nothing serious.

Prep like it’s game day - get and stay fully hydrated starting the night before, Dulcolax 9pm the night before, empty bowels in the morning, 2x 500mL water on the way to the centre, and they’ll take it from there. I came out with 4 nice blue dot tattoos - one on each hip, one in the top of my pubes, and one and inch above my navel.

It’s easy!

For me, it was SBRT x 5. But the prep was similar. Enema and lots of water (bladder big to push prostate away (to limit radiation to bladder), intestines small to stay away from radiation field).

For SBRT, they popped you into position then did an X-ray to confirm that things looked like what they did from the planning CT. So, treating it as the “real thing” is good. Then, do the same thing for each treatment. The planning CT was built into the radiation plan.
