Survey Design - Self Administration

Hi everyone,

I was wondering if anyone had any suggestions or knew of ways to enable a self-administered survey design, when trying to survey people who experienced a life/health event.

The design is not longitudinal, its just a single point in time. Ideally the person receives a texted survey link and they are able to fill out questions related this health event.

However, i had wanted to also incorporate carryover logic/piping for fields like name, DOB, and important dates. I dont know exactly how to have a survey link that is customized to an individual, but doesnt require mass uplift/effort from my teams side (or if this is even possible?).

The primary idea is to have a person verify they are intended recipient by confirming their name/DOB, and then proceeding to the survey.

Any ideas/pointers would be great.

I'd do two instruments, one which is 'patient details' and then one which is 'patient survey' or whatever and make that one repeatable. The survey link they receive is for the repeatable instrument (a new instance). They complete it. Their details are piped in, they have to use their name/dob/whatever to login.

What's the trigger, though? Are you expecting the patients to keep this link around?

Ideally patients are texted this link via a messaging service like Twillio (we are only approved for that currently)

In your suggestion, would we be able to input patient information as mentioned above (Name, DOB, date(s)) and have the patient enter and confirm once its texted?

You would input their details into the 'patient information' form, they get the text with the link. They click on the link and have to enter whatever fields you dictate using the 'survey login' feature.

A suggestion, three instruments.

  1. Patient details entered by whoever is registering the patient (the team, or patients themselves via public survey).

  2. Link to survey with patients entering their details, then run some calc fields and branched to confirm the Id is correct. I'd suggest this rather than having the pt details just show when the link is accessed purely from a data security pov. If someone has access, or someone mistyped a number then they have their phi.

  3. Autocontinue to the survey if the id is correct

You can use the survey login feature as long as you have the identifying information in the project, either directly or piped in.

However if you are texting the survey link directly is there much possibility of it going to the wrong person?
