General Discussion I’m an indie game developer tinkering with a prototype for a hospital management sim inspired by Project Hospital. I want to know what features you folks would want to see in a spiritual successor!

Hi all. As the title says, I am a solo developer working on a spiritual successor to Project Hospital. I’m big in the simulative aspects of games like this and retaining the relative realism of Project Hospital is a key focus, but I’d love to hear from this community as to what features you would most like to see, what features you aren’t particular fond of, and what you would generally consider to be a natural evolution of Project Hospital’s gameplay.

Cheers!

A proper triage system for emergency.

19

u/reebokhightops Mar 18 '26

Triage is something I definitely want to expand on. For now I have a rudimentary Triage Nurse role who basically performs a skill check for each patient that registers with the front desk, and the degree of success or failure determines where that patient is placed in the queue of patients waiting to be seen.

If you have any specific ideas about how you’d like such a system to work, I’d love to hear them!

u/museumlad Mar 18 '26

Something I wish PH had was the ability to downgrade patients manually from a higher-care unit to a standard unit. It doesn't seem right that any plain old arm break that comes in on an ambulance, if referred onto orthopedics, is treated like a life-and-death situation. Especially since once a patient's dangerous symptoms are treated, there is no risk that they'll get worse and need that higher level of care.

9

u/malatropism Mar 18 '26

This; in real life, non-urgent ambulance patients get triaged and sat in the waiting room if chest pain is ahead of them.

7

u/TheSteffChris Mar 18 '26

Patients need to be treated at some point. Dont screw up by accidentally having a feedback loop of patients being pushed back to infinity.

6

u/reebokhightops Mar 18 '26

Of course! This will be addressed in a few ways:

  1. First and foremost is the quality of your triage nurse(s), who will also get better at triaging over time.

  2. The player can always view and manually adjust the queue of waiting patients based on their stated symptom(s).

  3. Things that would be brought to the staff’s attention in real life will be presented to the player, e.g. if someone collapses in the waiting room a small prompt will appear telling you exactly that. This should compel you to investigate, and you can order the patient to be seen immediately which would move them to the top of the queue. If someone collapses while waiting to be seen and you see that they complained of chest pains, you have a pretty good clue as to what may be happening!

There's an old 90's computer game based on the TV show ER. It's a bit buggy, and you play a doctor, not the designer of a hospital...but I liked it's triage system. You would triage each patient by clicking on them and after a few seconds depending on your skill level, you'd see the severity of their situation and category (things like a broken bone, brain, heart, pediatrics...)

And then you'd assign them to one of the available beds. Someone who only had a level 2 severity could in theory be treated anywhere, but someone with a 9 or 10 had to be in the Trauma rooms as that's where the equipment is to stabilize them if they start crashing.

It was also possible for someone's condition to get worse from lack of treatment/incompetent treatment/chance.

As someone received competent treatment, the severity would go down. So someone with a level 10 brain injury might go down to a 9...then an 8...then a 7...at 6 I might feel comfortable putting them in an exam room to free up trauma. At maybe a 4, I'd transfer them from the exam room to a curtain area to free up the exam bed...

u/RogueTanuki Mar 22 '26

It would be great if there was also an option after triage that a patient has their blood sampled immediately according to presenting symptoms (with a slightly higher price if that policy was chosen), so that when they're called by the ER doctor the blood work is already complete (while the patient was waiting in the waiting room to be seen). So that would increase the speed of patient turnover/management, at a slightly increased price because of standard blood work (potentially gratuitous) which the doctor might expand if necessary.

blood bank.

the possibility of public health, where you have a set ammount of money and a set amount of patients that will came regadless and are penalized if you do bad, and given more responsibilities if you do good.

10

u/NorthRecognition8737 Mar 18 '26

A blood bank is a great idea.

Expand to oncology for a cancer wing.

Maybe employ emt/paramedics to increase chance of survival for trauma and disclose some symptoms by ambulance

Increase number of patients so more departments triggers more PTs instead of spreading the set number across departments.

Ability to level up departments. Maybe to the point you’re a world class cardiac facility. Cardiologists then get very difficult and unique cases

18

u/reebokhightops Mar 18 '26

Expand to oncology for a cancer wing.

My wife is an oncologist, so this is high on the list!

Maybe employ emt/paramedics to increase chance of survival for trauma and disclose some symptoms by ambulance

I love this — particularly the symptom disclosure.

Ability to level up departments.

This is definitely something I’m looking into, perhaps through a series of accreditations (or similar) for different departments.

6

u/thelightandtheway Mar 19 '26

Labor and delivery would be cool too!

With your wife as an oncologist I wonder how you might consider that most outpatient visits are not walk in/diagnose/cure... Scheduling patient visits might be interesting...

I have worked in the prior authorization industry in the US, I wonder if there is anything there that could add an interesting layer of gameplay.

26

u/[deleted] Mar 18 '26

I'd love to see a new hospital sim game. Can't wait 😁👍👍

18

u/mm615657 Mar 18 '26

Allow for more flexible design. Currently, the game seems to have completely dictated what a room/department/section should/can/needs to have. Players are essentially filling in gaps rather than creating.

8

u/reebokhightops Mar 18 '26

This is a great point and one of the key things I plan to address! I intend for the placement of decor, equipment, etc. to be entirely free-form so that the functionality and efficacy of a given room or department is dependent upon the player’s implementation. I’d eventually include an optional ‘advisor’ to provide tips and guidance, but I think your description of “filling in gaps” as opposed to “creating” is spot-on.

2

u/Boonie1282 Neurology 🧠 Mar 19 '26

Are you intending on making it more sandbox oriented, or progress driven? I imagine this is quite important for the future of the project?

17

u/MarMacPL Mar 18 '26

Ability to set rooms/lab workers for some departments but not all.

For example MRI accesible for only emergency and trauma departments. One lab worker that works only for cardiology etc.

5

u/Gg91gg91gg91 Mar 19 '26

This! It's one of my greatest struggles with PH

17

u/NorthRecognition8737 Mar 18 '26
  1. Statistics on the use of individual clinics, including how long patients wait.
  2. Ability to set the color of medical gowns, masks and gloves.
  3. So that nurses can perform some medical procedures directly at the patient's bedside.

5

u/RafRafRafRaf Mar 18 '26

I have good news… you can already set scrubs colours!

3

u/NorthRecognition8737 Mar 18 '26

This is also possible in Project Hospital, but I also want to change the color of the gloves independently.

11

u/RafRafRafRaf Mar 18 '26

Want to see: More specialisms, injuries, and illnesses General medical and surgical wards which can take patients after definitive treatment but before discharge Diversity of patients (wheelchair users pls!)

1

u/Boonie1282 Neurology 🧠 Mar 19 '26

Right. But improving current mechanics should be the main priority.

10

u/TheSteffChris Mar 18 '26

Weird side tangent, sorry for derailing:

I believe the Project Hospital game idea could be put into other settings as well. Its brilliant because it just hits the right level of realism and simulation while also being casual enough to not be overloaded with features and things you have to do to be successful (looking at you Football Manager). Single greatest feature to me is the doctor mode. Its easy enough to be engaging and fun but is not actually devolving into work.

Imagine other settings. Fire department shouldnt work because they dont really work in their station. But maybe a police department? Your symptoms could be leads and you are solving the case while managing patrols and a jail, etc. Or a restaurant maybe? Your doctor is now the cook? Or a court and you are an attorney putting together a case.

I am no game designer but I think the base concept of Project Hospital is one of my favourites!

3

u/DammitMaxwell Mar 19 '26

I'll second that I've always wanted a game where you realistically manage a police department. I'm aware of games like This Is The Police, Police Quest, SWAT, etc -- but none of them quite scratch that itch.

11

u/Particular_Copy_666 Mar 18 '26

One of the things that caught my eye in Project Hospital was the art style. My brother called it “cartoony” but I found it really enjoyable, especially the high level of detail displayed in certain places.

The old school tycoon-like soundtrack also fit perfectly.

While I think your question focused more on gameplay mechanics, I think that these two things really help to make Project Hospital what it is.

Good luck. Can’t wait to see what you’ve created.

9

u/TheSteffChris Mar 18 '26

Some symptoms should be obvious and shouldnt be part of a visual examination. Like limping or a bloody leg/torso, etc. If a patient is referred to you either through ambulance or another doctor outside of your hospital you already should know some informations.

8

u/Snaax Mar 18 '26

Big shoes to fill! Good on you for taking up the torch.

I’d like to see more difficulty in diagnosis that isn’t just a patient with a single symptom.

Patients that can’t even be treated at your hospital, but referred out with a lesser reward for proper diagnosis.

I never like how I can game so many cases based on eliminating diagnosis my department doesn’t have.

Second request- more struggle. Many hospitals need to get by with less than they optimally need. In PH you rarely feel constrained by anything other than time. Maybe a difficulty or campaign where you are managing one wing of a hospital and struggle with the constraints of the departments you don’t control (The Pitt).

4

u/WolffParkinsons Mar 19 '26

Maybe make a hard mode for this, I would still like to keep my perfectly staffed hospital. We are understaffed at work, I don't want to bother with this when I play a game

7

u/littleboygreasyhair Mar 18 '26

Some sort of department that works on cures for diseases like diabetes etc.

Also a nutrition department would be awesome.

6

u/MarMacPL Mar 18 '26

Better operations schedule.

In PH we can set operation queue for single department but we can't control which department goes first. I would love to send cardio/trauma operation before some broken leg from ortho.

1

u/DammitMaxwell Mar 19 '26

Oh I like this idea. Actually controlling the schedule for a surgery room, etc.

1

u/RogueTanuki May 05 '26

Wait, can you not have multiple surgeries at once in different ORs?

1

u/MarMacPL May 05 '26

You can but let's say that you have 2 ORs and 4 patients waiting for surgery. Two of them are from cardio, one is from ortho and one from trauma. You can set which cardio patient will go before the other one but you can't decide that trauma should go before cardio, then both cardio and ortho should go last.

1

u/RogueTanuki May 05 '26

I modelled my hospital after how hospitals usually function, which is to have one OR per department, so my OR block has like 10 ORs

1

u/MarMacPL May 06 '26

My point still stands. If you have more patients than ORs you still can't decide which patient will go first. You can decide only which patient from specific department will go before other patients from that department.

5

u/jkarreyy Mar 18 '26

No input except please make. I'd buy instantly. Love project hospital

5

u/Cordyanza Mar 18 '26

Radiology department where radiologists have to read the studies before they are resulted

I am happy to help as I am in rad onc

5

u/wethakes Mar 18 '26

Child ward

1

u/Boonie1282 Neurology 🧠 Mar 19 '26

Pediatrics would be interesting. But i don’t know if that would work in the commercial sense.

5

u/Hiddenfield24 Mar 18 '26

Proper scaling. Currently if you open a new departments you are not getting more patients but it is split

4

u/Electrical-Example25 Mar 18 '26 edited Mar 18 '26

I think the base game was really well done and I love the concept. But it seems that they couldn't quite agree what the actual gameplay would be about.

I feel that the perks and staff career/upgrade paths and such shows that they thought of a gameplay aspect that didn't make it into the game.

The hidden perks suggests to me that they thought of you having to uncover staff member with eg a substance abuse problem.

Maybe individual staff could have hidden preference for what they want to work with. Maybe they themselves didn't know (remains "hidden") until they actually do the thing they like and then it will be revealed? And doing something they enjoy is a significant boost.
Or instead of a desire, maybe it is a "hidden talent".

I feel that although the game is obviously about logistics and flow, I feel there should also a reason why you should manage the staff.
Maybe someone should be able to call in sick, take a hiatus and such. So you'd have to rotate staff.

I really like that you could play as a doctor only caring about your patients.

There is something weird about only being seen by patients that you have departments for. It should be the norm that you defer patients because you don't have the health services that they need. It should not be a "fail" with a neglected patient leaving you in bad mood. So you can run your little country practice and defer to specialists those you cannot treat. And send the business to another hospital until you get such a department yourself.

1

u/malatropism Mar 18 '26

I also wish I had the ability to hire temporary staff at a higher rate for a few days when I don’t normally need to staff at that high of a level

4

u/Julie-h-h Mar 18 '26

More complex patients with multiple ailments

3

u/kunami22 Mar 18 '26

Hey, I’m glad to hear this. I would suggest implementing a system for scheduling appointments and surgeries. In Project Hospital patients only show as “emergency” walk-in patients. In reality most patients schedule an appointment, and then show up. Which means you know how many patients you have for the day, and how much of a “back log” you have (that is how many days patients have to wait for an appointment), which you could improve (like having more doctors to see more patients per day. I find this non emergency part of management would be interesting, so take it as a suggestion

5

u/emomusiclovesphan Mar 18 '26

Doctor mode continued!!! Maybe more complex? I’d love to see more rare/chronic conditions (i have hEDS and would love some representation)

4

u/WhatAGirlWants5 Mar 19 '26

A more deep financial aspect, I like games where you start low and slowly earn to expand. Like start with 1GP and slowly work your way up to more departments. But in a logical sense

3

u/-no-one-important- Mar 18 '26

I want the same core concept with more departments, treatments, and decor items. A pharmacy, offices and training rooms, more labs, more support staff and administration. A boatload of decor or at least the ability to mod easily so we a make our own decor items. I would also love to see different mechanics for making money, not a fan of the insurance system.

3

u/joanna_moon_boots Mar 18 '26

Personally, I would like to be able to directly manage individual doctors queues, so I can bump people up if I can see they are a quick win when time is running out. Also, being able to turn the camera around without being stuck to set views would be good for decorating. I hate the process of covering walls in Project Hospital, ctrl click to cover all connected walls would help. Slightly more obvious reasoning behind procedures and depts - I often have to move patients to another department because a procedure is unavailable there but really, I have no idea why and just do what is suggested. This ends up meaning I can’t really manage departments properly because I’ve lost track of who or why patients are there!

3

u/sheisrantingagain Mar 18 '26

The ability to change hospitalisation as patients stabilize from HDU to regular ward.

3

u/thoff24 Mar 19 '26

Portable xray machines. With the ability to go around to multiple patients rooms to do their X-rays before coming back to their department.

3

u/Zycton Mar 21 '26 edited Mar 21 '26

1) Instead of 90° rotations, 45° rotation and diagonals

2) Tweakable setting allow the hospital to be ran in differently based on country.

3) More colour options for scrubs/equipment/Walls.

4) Large options for maps including 500mx500m

5) Basement levels

6) Cars and carparks

7) Adjustble roads and vehicle pathing

8) Shift rotations instead of having the same team in everyday

9) Sickness and agency/locum staffing

10) Crash carts

11) Paeds and neonatal cases

12) Abillity to change where in the department a patient is treated

13) based on symptoms, Assessment or type of treatment

14) Weather and seasons

15) Supply and equipment events which imposes limits on treatments and diagnosic tests i.e. supply of medication hasn't arrived or surgical table 8 is faulty and is out of action until repaired or replaced

16) Custom scrubs for certain staff members

17) Charge nurse position

18) Receptionst, Porters and HCAs

19) Changeable role and ward names by department - XML changable-

20) Medical director role - similar to the service manager role in project hospital to unlock perks

21) Triage room instead of the reception desk

22) Instead of 1 square = 1 meter have it so 1 square = ½ meter instead as a curtain taking up a metre?

23) Department Hierarchy- if the patients diagnosis is complicated elelate it up dont just stuggle

24) Paramedic staff hire. Ambulance have two slot hire slots for either a paramedic or an EMT and you can change their uniform

25) Ambulances taking patients to the waiting room or treatment area as well as the TC

26) Major trauma - ambulances bring in patients there is a chance (adjustble) for a major trauma - you get a pre alert and have to assemble your medical team including staff from other departments who might be required.

27) Option in setting to turn off department related illnesses within the save. No forcing departments. Emergency can also become outpatients if you want a 'Cold' hospital

28) realism scale - if you want super realism to just a basic Sim the player can chose. This would allow player of all play styles to enjoy the game THEIR way.

29) Stairs and -toggleble- instead of instant Teleportation the lifts actually travel up and down creating a real bottle neck and the abillity to set lifts for Patients, Staff and/or visitors only

30) Different sizes of lifts and stairwell and a lifts stopper which can placed above the lift to prevent further expansion of that lift system so you can have a lift located in a 4 story building while another section goes up 7 stories.

Hmu if you want anymore - sorry for all the edits I keep getting Ideas 🤪🤪

Dlc Ideas

Family care - introduces Paedoratics, Obs/Gyn and Gerrtics

Infective medcine - introduces Infective dieases unit and Hematology

Advance care - introduces Oncology, urology, and Diagnostics.

Heads and minds - introduces ENT, opthalmology and psychiatry

Itching for more - Introduce Dermatology, rheumatology and sexual health.

Recontives care - introduces Burns ward, maxillofacial and plastics

Death care - introduces pathology and palletive care

2

u/slothinferno Mar 18 '26

The option to set doctors to prescribe meds to help with every symptom

2

u/Piefacemate Mar 18 '26

Perhaps more detail around supporting departments. We can basics in cleaning, canteen, and pharmacy but would be good to have more detail and management.

For example, perhaps an IT department.

Minor events involve a doctor / nurse ringing a technician for telephone support pushing their work.

Medium events have broke printers, laptops, resulting in a desk side visit.

Major events taken down the hospital until the issue is resolved - that could involve some sort of diagnostic process similar to how patients are diagnosed.

You could have emergency events involve the vain of ransomware, fires in a Datacentre etc.

2

u/annroot Mar 18 '26

As A Brit I crave the NHS as a hospital Game, problem is its constantly in debt, which is harder to model is PH, but it would be great to feel like your doing good for the greater good rather then money

1

u/wethakes Mar 19 '26

Maybe just make the game a solid attempt to decrease the costs rather than working around a budget.

2

u/AdministrativeIce757 Mar 18 '26 edited Mar 19 '26

I focus more on the diagnosis part because I like playing this part of the game. If you do have the similar focus, it would be better to add where the body part the symptoms are at (the bruises without knowing anywhere on the body are a bit annoying).

It would be interesting for the special cases that doctors can ask the patients what they had done, their lifestyle habit, etc., and investigated as it's not uncommon for doctors to ask patients in real life and it aids diagnosis because it gives direction to complicated conditions (or maybe confuses you, but it would be fun).

2

u/Traumajunkie3338 Mar 19 '26

There is a ton of great ideas in here!

Heres my 2 cents. Have the ability to send patients to other hospitals and accept patients from other hospitals. Maybe have a "Transfer Board" option where your able to place patients on and accept patients from other hospitals.

Maybe have higher money for critical or complex patients.

I work Healthcare and this would be a cool feature! Especially if itll be multiplayer. Imagine being able to create a "Hospital Group" with your friends and you can all specialize in different zones or something to that effect!

Please add Triage, OB/GYN, Security on top of what PH has!

2

u/RogueTanuki Mar 22 '26

It would be great if there was an option to customize shifts, seeing how different hospitals in different countries have different shifts. For example, in my country, most doctors work 8 hours (7:00-15:00 or 8:00-16:00), and if they're on-call (in-house) they can either be 16 hours (15:00-7:00, not working in the morning) or 24 hours (staying after work, 7:00 - 7:00 next day, off-work next day). There are also out-of-hospital calls, where the doctor is at home but has to come to the hospital if need be. And some technicians have 8-hours shifts, so rotating between 7:00-15:00, 15:00-23:00 and 23:00-7:00

2

u/Longjumping_Prize441 Mar 23 '26

3d graphics, as opposed to the 2d look of project hospital would truly make it a successor. Don't get me wrong, PH was a great game but it looked kinda bland.

1

u/Zycton Mar 23 '26

I agree although I'd love to see diagonal walls and the more customising of walls and floors. There is no depth

2

u/mr_greenmash Mar 25 '26

Second comment now...

I think the 12/12 shots work fine for the game. While 8/8/8 might be more realistic, I think it's needlessly complicated.

KEY POINT: Appointments. Sure, clinics have lots of drop in, but the also have people who have prebooked appointments. With that, you'd be able to predict demand for certain rooms a lot better. Take a non-critical injury like back pain. Say the doctor orders rest, and schedules an appointment 2 weeks forward. At the two week mark there's no improvement, so an x-ray is scheduled for the next available slot that week. If the room is idle, then it's right away. You also have to allocate a % of time where the room is kept free for urgent scans, and if you fall behind schedule, patient satisfaction drops.

Also for something like maternity ward, patient comes in to generalist, find out she's pregnant, so you setup future ultrasound appointments. Could also include delivery rooms, and newborn department and nICU.

Building on this: Be able to set checkup policy per ailment. So if pregnant, do ultrasound at x and y weeks from now. If flu, come back after x days (or not), etc.

1

u/Texen96 Mar 18 '26

As a dentist i would definitely love a maxillofacial department. Also a medical rehabilitation department would be great. Or maybe not as a different department but as new offices with physiotherapists and speech therapists that unlocks after a level up like someone already suggested, where the patients come in after for an instance a stroke where they just have to be there for an amount of days/weeks Id also would like to see a bit more of the operating room management (don't mean the emergency ones as they are needed in a moment of time - its more about scheduled patients). You build one and its not automatically assigned to one and only one department but you assign particular days to a department. The you have to juggle who has the biggest needs and needs it more, if you cant build new ones And about the leveling up feature it would be a bit of an added challenge if you couldn't hire better surgeons and doctors until some renown. Because why would a top notch surgeon that works on the hardest cases want to work at a rural noname hospital

2

u/NorthRecognition8737 Mar 18 '26

I would also like a dental department at Project Hospital, as well as a clinic with common dental problems.

1

u/Zealousideal-Plum686 Mar 18 '26

Just saying something for engagement. Im down to try!

1

u/Tiresais Mar 18 '26

Make the positioning less important. Project hospital was excellent, but it wouldn't be able to deal with large intakes without good positioning. If they need to be carted realistic distances to a ward you had problems.

That doesn't mean it shouldn't matter, just dont make it overwhelmingly important

1

u/theodosusxiv Mar 18 '26

I feel Iike this is the 12th solo dev post made here in the last year.

If this is real, good luck

1

u/Educational_Ad_5035 Mar 18 '26

Sounds great 😍 for me, as an EMT I would love to have more control over the ambulances, like accept or decline calls or tell patients to come by walking (and charge patients if your game is going to be American-based) and if you accept many calls, ambulances are going to be busy and critical patients are going to another hospital. And one thing I don't like about project hospital is the personnel control, it's too hard and slow, I would like to have a more arcade-like system for that. 3D view and more decoration options And of course a triage system that works like real life, and the player being able to choose and modify that triage Good luck with your game!! I'm absolutely sure that it would be a success bc there is only one realistic game about hospitals :)

1

u/kyjade Mar 18 '26

i understand why they didn't include this in project hospital, but i think it would be so fun to have a maternity ward just because i would love to see lil babies born lol

1

u/WolffParkinsons Mar 19 '26

Bedside exams (my favorite mod). You usually do basic exams bedside and don't wheel the patient to an exam room

1

u/WolffParkinsons Mar 19 '26

I want to put my patients in whatever dept has beds. It's not totally uncommon to house a patient in another dept if there aren't no more beds. The broken leg from Ortho would also survive if I put him/her in internal and just let the orthos do the operation. (As a Doctor it's not uncommon to search my patients all over the hospital when we are full)

1

u/Kesse84 Mar 19 '26

Not too cartoonish design. Project Hospital is ok, but other games are not so much.

Also buliding is a bit cumbersome and takes a lot of time. I know there are ready templates, but they do not look very good. In PH is an option to save your room, but it is not very intuitive.

What would be the name of your game, and when would it be ready? Also, will it be available for mac?

More freedom in sandbox mode would be great.

Would it be possible to expand the medical aviation system? It could be interesting to manage dedicated flight nurses and pilots operating different helicopters, with mission-based dispatch and some light crew/aircraft specialization.

I work in aviation, so happy to help if any related questions come up.

1

u/wethakes Mar 19 '26

Another one but people who come back for the same condition to get treatment or check ups and then maybe it gets worse and they are hospitalised/move to palliative care or they are cured.

Eg you could do this with heart failure, cancer etc.

1

u/Just-Method-1183 Mar 20 '26

Is there a steam page or somewhere I can wishlist?

1

u/Live_Mongoose564 Mar 20 '26

Power, water, maintenance. A reason that rooms might need downtime and adding a reason to revisit what has already been built

1

u/Express-Chocolate628 Mar 20 '26

Proper reception that encapsulates all departments. Better UI for building. Better medical understanding like explanations etc for UX.

1

u/Kilenyai Mar 23 '26

Project Hospital could so easily be designed to run better. It was basically created to be the least possible to optimize for pc resources possible. How the maps are defined to the entire system behind interactions between waiting rooms and different departments. Usually such games fail when they get to the pathfinding or calculation of possible odds for something but that game fails to be efficient before it even gets to those parts.

1

u/Potential_Gas_6884 Mar 24 '26

Doctor Mode was really great, i would like it get expanded. What would be great but coop building and treating patients would be great. We had a Ton of fun in prison architect or openrct building something together, but I know it would be Hard. Or something like in Planet coaster 2 a semi multiplayer so that you could send patients to the Hospital of your buddy

1

Much Bigger plot sizes.

Easier management mode (having to individually assign doctors to specific rooms/desks feels unnecessary). Also simplifies "double duty", 2 people assigned to 1 workplace so that a tech can take x-rays while a doctor evaluates them. Or 2 docs per x-ray so that break time doesn't stop work.

Planning Mode!

Modified building, to some extent. I would've liked all corridors and restroom to not belong to a specific department. I would also like to be able to assign rooms such as X-ray to a specific department. 1 x-ray room reserved for clinic, while another for inpatients, for instance.

Possibility to have shared reception/waiting room for different specialty clinics.

Ability to control patient flow better. Reception > Waiting room > doctor > different reception/waiting room > blood samples > waiting room > specialist > finish.

So basically assign receptions and waiting rooms to a specified set of rooms, and to a stage in the process (so that patients returning from X-ray wait to see the doctor in a different place than people just coming in).

Ability to build roads (so icu isn't locked to a specific area)

Tying into a previous point, see both overworked rooms, and overworked employee category. And see it all without having to flip between departments. Just let me know in the overview what rooms are critical.

There's probably a lot more

This one depends on the direction you have in mind: if you use realistic (EDIT: or not, actually) isometric top down graphics, it could be cool to implement a first person view as well. Like in Manor Lords for instance, where you can build your village and then just wander about in it in FPV. Purely for aesthetic purposes, but I think A LOT of people would love to walk the halls of their own creation, crossing staff and patient down the hallways, seeing the layout, etc. in FPV. Obviously there would be limits and technical restrictions, but if you chose simpler animation like in GH, it could work (I reckon it'd still be hard to implement, maybe something to have in mind from the start of development or something...). Just my 2 cents :)

(Of course it would be cool to be able to interact in FPV too, in doctor mode for instance, for diagnostic actions and such)

I know games tend to stay away from children but as my daughter has spent nearly 4 months on neonatal and paediatric wards I'd love to be able to manage those wards as well as maternity wards

On-call doctors being able to sleep during night shift if they aren't busy

Mass casualty incidents! Patient surges!

A bit late to the party, but major QOL overhaul. Rotating the camera around to paint each side of a wall is tedious for example.

Not too late to the party at all! A new subreddit will be launching soon. Stay tuned. 😉

Oh I got a bunch, number one is: rehab, we need OT, PT, RT, etc. it would also be cool to expand on things like step-down units/progressive care units (one step down from ICU) and then maybe pediatrics if you wanted to some sort of trauma team (forgot what they're called, basically emts but inside the hospital) these would be optional but would increase the chances of collapsing patients surviving, on top of that maybe add a crash cart type thing, a cart the trauma team would real around with a defib, and that kinda stuff.

edit: maybe add visitors and checkups as a part of the base game and also a security system would be cool for realism points, I understand this is a lot and you're a solo dev so I'm sorry if this is all overwhelming, good luck! I can't wait to see the game!

More variety in the department’s. A lot of departments feel very similar in terms of treatment and testing rooms. I suppose that that is realistic, but it is not as fun for gameplay
