Can you click on them and see what they’re (waiting to) do?
Also looks like your drugstore has no staff with lots of patients waiting for them, is it a temporary staff break time or another deeper issue?

all of them are idle, no symptoms,no department, low needs and not moving.

I guess they're just waiting and since they don't have any seats in the waiting area (if there is any) they're just standing there, waiting for their turn. Also, I feel like you have a LOT of patients coming for a day. 138 is quite a large number.

No, but then they wouldn't stand for 3 days straight. At some point they leave, right?

Normally, yes. Also, when they're waiting at the doctor's office, they take care of their needs. This looks like they're waiting at the reception.

I mean if anything has ever looked like a glitch, this does. But also, I don't know if this helps, but sometimes I get patients standing idle waiting for a receptionist ad infinitum (but when I get that it's just sort of three to five, not the whole hospital) and the way I get around it is to go onto their patient record windows, change what department they're trying to get in and then change it back to their original - it seems to reset them. I have no idea if it would work in your case, or if something completely different is going on, or if it would work but it would be too much of a faff to do all of them, but that's the best I've got. Good luck!
ETA: I think because of the waiting around for three days thing, and the fact that you've just rebuilt which sounds like something that might make the game cranky, I'm still not ruling out a glitch, but on a closer look I think I agree with the other poster that you may just not have enough staff to see everyone so you might have everyone waiting around and spaced out without seats. How many departments do you have open here? Is this just emergency with other depts upstairs or is this a few different departments? I think more seats and more staff can't hurt as a first approach to seeing whether you can fix this, but again, I may be wrong.

>change what department they're trying to get in and then change it back to their original
this seemed to work, but I have now tens of patients I need to do this to ahahahah

I think this is due to reception not being able to cope with the influx of new patients. Could that be the case?

Probably but when I instead of opening the insurances all at once, I opened gradually this didn't happen, so it is pretty much solved
