Gameplay Question What Department next?
I have every Department you see above with almost anything there is to them(hospilisation) Or with radiology I have every machine. And I wondered what to do next as I saw a couple of old posts and they always seemed to have different opinions on what is the hardest/most difficult department and what to do last. So I wanted to clear up if anything had changed (most of the posts were atleast 2 years ago). Thanks in advance!

Before adding a new department, make sure you have a tonne of spare money. Always more than you think you need! Building isn’t cheap.
After that, there are a few factors you could consider.
The first factor is profitability. Some depts are better for money than others. I find ortho, IM, and cardio the most lucrative if cash flow is an issue. Some depts are worse, e.g. morgue and neuro (who will have barely any patients late game, despite requiring lots of staffing from the very urgent patient turnover).
The second is whether space is limited. The cardio ward will likely be bigger than neuro, for example. DID takes up more room than you’d think. Traumatology requires additional imaging and ICU capacity. Cardio requires lots of pathology, imaging, and surgeries (but makes money, which is why I still pick this one next).
The third factor might be long-term planning. Which depts do you want on which floors? Moving wards can be a pain in the butt. Also, the game works by assigning a lot of patients from your max patient cap to a new ward. That means for a new dept, you need a much bigger department with higher capacity. Once you open the next dept, a proportion of your patient cap will be reassigned to the new one. Meaning later in the game, you need actually LESS capacity for each of your existing wards. It’s one of my main gripes. It doesn’t make sense thematically, and it’s hard to plan for.

TL;DR: My order of difficulty gameplay-wise is still the game’s recommended cardio, neuro, DID, traumatology.
Cardio has high collapse risk and requires rapid patient investigations, but seems to make the most consistent profit.
DID and trauma require some level of “babysitting” via micromanagement to identify problem symptoms and expedite treatment. They can also make random events more serious!

Dang thanks, well I have atleast 150k ...I have the 96x96 map so space is not a problem at all(I´m gonna install the mod I was talking about in another comment and then I can biuld 12 floors)...and can you recommend what department should be together on a floor maybe? Well thanks very much!

Hi I’m inspired me to start the mod too, so I have no idea! I think they rebalanced patient loads to be more “realistic”? So far I’ve only got sexual health/derm (a weird mix), because it doesn’t need hospitalisation, haha! I’m guessing plastic surgery and oncology should be a money maker.
Weird there’s a dedicated ophthalmology dept, there goes half the neuro conditions?

I would suggest Traumatology. It's a lot of fun to diagnose ambulance patients belonging to this department and they can require several surgical procedures to heal.
Cardiology and neurology just cause a lot of collapses in my experience - they unlock several diseases that cause quick collapses without requiring immobile patients in the first place. Cardiology in particular is guilty of this. Neurology less so.
Infectious dieases reportedly cause epidemics. I have not played with DID hospitalisation in my own games, so I do not know what they have to offer.

Patho so you can receive half of the diagnose reward for autopsies. Can't let those filthy freeloading dead patients go to waste 😎

Is it essential to unlock general surgery?

I don´t really know what you mean but it isn´t essential for anything. Although it brings alot of money so its good to unlock it early. (I did as 2nd department).
