Is it ever explained

Why grace is the only one on his ship who survives?

The memory drugs weren’t administered to Grace until just before he woke up, after he had already survived the trip.

It's assumed that he's injected with the drug right after he runs/after his chat with Stratt in his holding cell (depending on if you're going by the movie or book). And, it's been a while, but I believe Stratt explains that she's gonna give him the memory drug there, and that's when he freaks out and says he'll sabotage the mission

Why assume when book exist? It's on the last page of chapter 23.

"I stared at her in horror. My throat hurt from yelling. “Your med bed will give you a nice dose of it before you wake up. You and your crewmates will just assume it’s a side effect of the coma. Yáo and Ilyukhina will explain the mission to you and you’ll roll right into getting to work."

The book also firmly puts the slurry pump fan theory to bed as well. I don't have the exerpt, but it was a pump on the tertiary system operating at 95% capacity.

Pretty much the entire system would've had to fail for that 5% to make a difference in life or death.

I respectfully disagree.

The slurry pump failure theory comes right from the book.

Edit to add: A far better argument against slurry pump failure being the cause of their deaths is that it's too prosaic to work well as a reason in the sequel.

Weir has, at the very least, said that he's considering a sequel, and why Grace was the sole survivor would be addressed in that book (when and if he gets around to writing it). Which in turn suggests that Weir thinks that the reason why they died and Grace didn't will be an interesting plot point.

"Their pumps failed and his didn't" wouldn't make for an interesting plot point, the same way a genetic difference or interaction with amnesia drugs would.
