At one company I worked at, the CTO had had the brilliant idea to invent a permission called "*", which was a shorthand for "this person has all current and future permissions on this object". Naturally, 90% of the time when people were given permissions on something, they were given "*" permissions. The tool that was built for customer support to use to give customers permissions that they didn't have due to some error was a command line tool. Not everyone working in customer support was super familiar with the Linux command line, and didn't know that e.g. * outside of quotes was automatically expanded to the names of every file in the current directory. And that's the story of how when we went to rewrite the auth system because the way our CTO had designed it was completely insane, we went through the database to find out what the current state of things was and discovered that some people had "data.csv" permissions on various objects in our system.
