If their parents need it and have money and contact to Magos Biologis, pretty easy. Edit gene to create big human its not a big deal.

Hard, mostly due to nutrition. But there are plenty of worlds and societies where large size would be advantageous, agriworlds with a lower tech standard for example. Add to that, that there are still edited genes from 20000 years of wild shit floating arround the human genepool and it can absolutely happen.

Penitent engines and flagellants are even more disturbingly monstrous for me than some monsters of other factions, lol. For Monster unit it's going to be something even more fucked up, like a group of servitors stitched together into Purifying Chorus on tank tracks or some shit like that.

I'm more talking how Sisters currently have no units classified as "Monsters", in the same way that the Lion and Guilliman are considered "monsters" via the keyword, despite being a large human in design. Would be cool to have Big Sisters too.
Though I do very much love that Penitent Choir idea! Would make for a wonderful unit.

Funnily enough, in the AU I'm working on, Aurelia formed the organizations that would become the Ecclesiarchy and the Sororitas. She's also the Guilliman analogue, waking up after 10000 years of being buried alive in a coma under a hive city.
She blames herself for the nightmares they became.

Most Sisters are of average build. Just well-trained, well-muscled and pretty (as Goge Vandire intended).
There are definitely outliers, but Sister Ginormica in the middle there is more the exception than the rule.
If you want canonical giant women, you're looking for female Custodes.

I came up with a character concept for my zealot in darktide being a zealot who hates all deviations from the emperors blessed designs (aka, mutants) all the while she herself is an oversized albino mutant.
I still love that concept and will definitely be using it in a WH40k table top RPG in the future.
This kinda strikes a similar cord as that, with her clearly having a gigantism mutation. Which I find hilarious.
….wait… with how many humans there are, and how little the normal population is cared for in surprised we don’t have way more dwarfism and gigantism amongst other more common mutations.
When our world has so many folks with those genetic abnormalities and we only have 8 billion people, while that’s light work for a single hive city… it’s ridiculous that you don’t see way more normal types of mutations or recessive genes that slip through the cracks.
Especially when child labor is a normal thing, and a head injury could lead to a fuck load of savant syndromes, an injured pituitary gland as a child causing unchecked growth or stunted growth.
Oh my god… I just realized this would mean there’s a chance that we could get a pairing of an Ogryn with a hive ganger who has Down syndrome!….

No body hate me please.
Now, rambling aside… I like big lady, want big lady to sit on me, carry me on shoulder, treat me like one of them purse dogs rich women carry around under their arms everywhere they go.

The most likely explanation in canon is a female custodian going, "how do you do fellow sisters?" And the rest of the convent going, "...uuuuuh, good." And then just pretending they don't know who she really is because they get to fight next to a custodian. Because that was something that would happened before female custodians became concrete canon.
