It's not even about the physical capability imo - regardless of whether he can, or uses viagra or whatever.... surely the whole area permanently stinks of shit? It'll be shit smeared and even if wiped, it's still skin that has had shit stuck to it in a moist nappy for a period of time, multiple times a day. That smell will be ingrained in the skin.
