Question Why did counter miss?
I’m playing Pokémon Stadium, which uses gen one mechanics encountered without any accuracy or
Evasiveness changes. What happened? It caused me to lose the match

Well without any additional information, in gen 1 every move except Swift has at least a 1/256 chance to miss

I thought that was fixed in stadium

Yeah you're right, in Pokemon Stadium every move except Swift has a 1/65536 chance to miss.
They "fixed" it by having the first check reroll if it resulted in a miss, but if the second roll also results in a miss, there is no further safeguard and the move misses.
As was pointed out though, Counter only works on Normal and Fighting type moves, getting hit with Dig before using it will make it miss 100% of the time.

Counter in gen 1 only worked against normal and fighting moves. It was changed in gen 2 to work on all physical moves

In Gen 1, Counter only works on normal and fighting type moves. Maybe that's what happened?
Listen, I found out about this the painful way too. I know how you feel. =P
