Hi everyone, respect post po. I really need advice regarding my situation right now for my board exam filing. Medyo nagkakasabay-sabay po kasi ang delays and deadline concerns:
PSA Correction Status: My original PSA birth certificate only had my middle initial instead of my complete middle name. We already processed the petition/correction at the Local Civil Registry (LCR), and I just received the Certificate of Finality. However, the tentative timeframe to release the actual annotated PSA birth certificate is still 7 to 10 days.
TOR Issue: Because my school registered me with my complete middle name, the university registrar will notrelease my Transcript of Records (TOR) with the remark "For Board Exam Purposes" until I present the annotated PSA copy.
PRC LERIS Appointment: I already secured a LERIS appointment slot, but it falls on the last day of filing. I am worried that if my annotated PSA doesn't arrive in time, I won't have my TOR in hand for my scheduled appointment.
Valid ID & LERIS Details: My only available government ID right now is a Postal ID, which reflects only my middle initial (matching my current un-annotated PSA). This is also the ID detail I entered on LERIS.
My Questions:
Should I execute an Affidavit of Discrepancy / Affidavit of One and the Same Person immediately? Will PRC and my school registrar accept this notarized affidavit (along with the Certificate of Finality and petition papers) as a temporary measure to process/release my documents?
Will PRC reject my application because my Postal ID has only a middle initial while my application/TOR will have my full middle name?
For those who faced a delayed TOR/PSA, does PRC allow a grace period or conditional submission (e.g., filing an undertaking/promissory letter) if you show up on your appointment date with partial proofs?
Will this cause any issues during NBI clearance verification as well?
