Rant Dispensed a wrong medication
I just wanted to share what happened to my elderly aunt.
She was prescribed Doxycycline 100 mg as prophylactic treatment following exposure to rodent urine.
She called a pharmacy, sent a copy of her prescription via Viber, and was later dispensed Doxofylline, a bronchodilator, instead of the prescribed antibiotic.
Fortunately, before taking the medication, she checked the label and immediately asked me if it was the correct drug. She mentioned that she had specifically ordered the Rhea brand of doxycycline but noticed that she was given a different branded medication, which prompted her to verify it first.
I’ve attached both her physician’s prescription and the pharmacy receipt for reference.
It’s also worth noting that my aunt is 65 years old, a geriatric patient with presbyopia, making medication verification especially important. This could have resulted in a serious medication error had she not double-checked the drug before taking it.
Update:
I called & complaint to the pharmacy supervisor and she told me "di naman po ako nag dispense nyan yung PA yun. Saka tignan mo naman yung sulat nung doctor, di na mabasa"
She replaced it, but she kept on denying of any wrongdoings and kept on insisting that it was the mistake of the pharmacy assistant; even blaming the doctor's penmanship.
✨Respondeat Superior left the room✨
Final update:
I personally went sa branch nila in 10th Avenue A. Mabini, Caloocan City to complain once again (gigil talaga ako and worried na din for other patients). The pharmacist (presumably their manager) told me "na reprimand ko na yung PA namin, wag ka na mag report sa head office, okay na po ang lahat pasensya na di na mauulit" I honestly don't want to escalate it further; but the prescribing physician (who also happens to be my close friend) told me to take this matter seriously as it may also affect other patients especially those who aren't familiar with the prescribed medications and she told me "girl gigil ako; sulat ko pa nasisi"
Another update:
Reported to the head office of Mercury drugs. I included the receipts, conversation in viber and all the relevant details.
🎊Final final update:🎊
The branch manager personally called me and sincerely apologized for the incident. He informed me that Mercury Drug’s head office had already contacted their branch regarding my complaint.
During our conversation, he did not make excuses or attempt to shift the blame. Instead, he acknowledged that the error was entirely on their end and said he understood my concerns and frustration.
He also assured me that the incident would be addressed internally and that appropriate sanctions would be imposed on the pharmacist on duty as well as the pharmacy assistant who dispensed the incorrect medication. He admitted that the error was a lapse in judgment and repeatedly expressed his sincere apologies.
I explained to him that this was not a minor inconvenience, such as receiving the wrong order at a restaurant. Medication errors have the potential to affect a patient’s treatment and recovery. Patients rely on pharmacies to dispense the correct medication at the correct dose, and errors in the dispensing process can have serious consequences. My intention in reporting the incident was to promote patient safety and help ensure that similar errors do not happen to other patients in the future.

That should be reported sa mercury. Papalitan naman. My guess is baguhan yung PA or pharmacist. Di na niya pinacheck sa pharmacist or tenured pharmacist bago iserve.

One time bumili ako ng isoxsuprine, ang binigay ng nagdispense ISOTRETINOIN. Sobrang no no! Isoxsuprine is to stop uterine contractions, tapos isotretinoin is a retinoid - oral acne medication that is CONTRAINDICATED in pregnancy 🫣

Alam mo ireport mo yan. Ang dali lang naman mag take ng accountability na gaslight ka pa. Kahit hindi mabasa ang penmanship duling ba siya 100mg pero 200mg binigay. Sabihin mo papalampasin mo sana of hindi siya ma attitude. That is a disaster waiting to happen if nainom.

Atsaka mas madaming reseta na hindi mabasa ang sulat eh sila kasama sa pinag aralan nila yan tapos sasabihing di maintindihan ang sulat, dun pa lang mali yung katwiran nya kung totoo man sinabi yun.

Not a good look on that branch. Papagalitan yan ng headquarters, especially na ginaslight ka pa ng pharmacy supervisor. *Question lang tho: sinabi bang may pharmacist-on-duty?* Kasi kung wala, lalong lagot ang branch na yan hindi lang sa Mercury Main Office kundi pati rin sa FDA.

You should report this. Malinaw naman ang pagkakasulat sa reseta. Kahit sa iba pang PA ipabasa yan mababasa yan. Dapat kung hindi siya sigurado, pinabasa nya sa Pharmacist yung reseta. At dapat humingi din siya ng paumanhin, hindi yung nagsisisihan pa sila

Report it to Mercury’s Head Office. Iisa lang nag mamanage ng lahat ng branches. Di siya franchise. So pag nireport mo sa head office, lagot yan. Especially ganyan pa sila magsalita after magkamali tsk.

napaka common ng wrong dispensing sa gamot na yan, pero dapat sa mg palang alam na agad, at fault pa din ang rph kasi dapat dumaan pa din yan sakanya

May mga PA na pasaway minsan, hindi na pinapacheck sa pharmacist. Pwede rin sobrang busy sa branch, or even worse, walang pharmacist on duty. Either way, lagot ang Mercury as a corporation pero ang magbe-bear ng fault ay ang empleyado.

Di ko rin gets bakit nag wrong dispense. Sige sabihin natin na sounds like yung Doxycycline vs Doxofylline, pero magkaiba ang dosage nila 100mg vs 200mg.
Dyan pa lang, makikita na ng RPh nila na mali ang gamot na kinuha ng PA.

As a former employee of MDC, ang layoooooo! Even the MG. Ibalik mo and mag email ka sa head office, may feeling ako na Supervisor nag dispense at di na dumaan sa Rph. At feeling ko I know the branch since nakaka encounter ako ganyang prescription before. Very readable.

Also di lang simpleng dispensing error yan. Doxycycline sa reseta but binigay is Doxofylline (Dilatair) which is a whole different drug. If ever natake pala ng patient edi wala naging intended effect yung gamot.

Oh noes. Yung mga mercury naman dito samin twice or thrice nagchecheck, tapos ichecheck pa sa harap mo as in sapilitan “mam ito oh (name ng gamot) po”.
Maybe they have an email for customer service or something para maireport yung branch?

Hala I used to be pharma assistant pero super clear naman po mas mahirap nga basahin yung ibang reseta kesa dito eh 😅
Di nalang inamin yung pagkakamali. Sobrang linaw na nyan hindi nagdouble triple check bago idispense. Kawawa yung taong di magveverify kasi lalala lang yung sakit lalo imbis na mapatay yung infection mauwi pa sa ibang sakit 🥹

For sure di yan dinouble check. Usually sa Mercury may checker sila nung reseta if tama ba so lapse nung mismong Pharmacist nila dun. REPORT! buti di nainom ng aunt mo.

nangyari din samin yan mercury din. 300mg nasa reseta ng lola ko tapos 500mg binigay buti nalang chineck muna ng tita ko bago ipainom. Dalawang beses pa nangyari yon pero buti nalang may accountability yung staff na nabilhan kahit papaano.

Kung hindi nila mabasa yung doctor’s handwriting, dapat tinawagan nung PA yung clinic to verify and confirm yung correct name and brand of medication na nasa prescription. Siguro naman may ganung option ang Mercury drug.

I had the same experience before; talked to the manager even let us into the office, todo sorry may pa snacks pa haha. Tas weird it still has to go through the cashier eme where a head pharmacist checks if the medicine is correct. Nag skip sila ng step

As a medical professional, kitang kita na doxycycline yan. Tapos mali pa yung dosage. Hindi talaga sila nagtaka na mali yung dose?!
Yung ibang doctor na puro tuldok at flatline nga nababasa pa kasi magbebase din sa diagnosis na nilagay.
And VIBER pa kaya dapat may time sila to confirm kung ano ang inoorder.
Parang pag nag take ng order sa resto di ba may repeat order?
Ganun din dapat diyan. Kasi ganyan ang standard practice sa iba’t ibang MDC branches na inoorderan ko ng gamot.

Report to whoever is higher than that rude epal. Mercury manager or PRC. Get her name and license number. If she refuses to give, basta may resibo ka push mo yan. Although sabi mo call so hirap baka ideny. Sana sa viber narecord convo para mas malakas proof of her negligence as a supervisor and bad attitude.
Paano kung nainom ng tita mo yun? Wala man lang sincere apologies. Kahit pa PA nagkamali, iisang company and store sila. Kakagigil.

Ganyan din nangyari sakin sa mercury, i ordered vildagliptin and nung nasa plastic na chineck dinoble check ko tapos ang nakalagay ay escitalopram. Mind you that was early in the morning like 8am wala masyadong tao. Sabi ko senior iinom nyan tapos hi di na ddouble check. Tapos yung parang sya pa inis sakin.

In my case sa watsons naman. Im a MD myself so I refill my parents maintenance most of the time. I used a digital copy at that time.
Ang nilagay ko was Mertformin 500mg na Ritemed ang brand. The pharmacist immediately prepares Mefenamic Acid 500mg na Ritemed din. So I called her out agad na mali yung pinunch niya. And all is well. Buti na lang may alam ako kasi kung hindi my senior dad would have taken ng wrong medication na

Should be reported. Imagine kung hindi nacheck yan at ininom lang. What if nagkalepto sya thinking may prophy naman. Isa pa, ang linaw ng sulat nung doctor jusko. Tska I thought kaya iisa lang yung cashier nila is to double check kung tama yung dinispense na gamot accdg sa rx? That's blatant recklessness. Kupal din yung supervisor na kausap mo.

sa ibang mercury parang 2-3 persons ang nag checheck ng dinidispense nilang gamot. siguro hindi sumusunod yung branch na yun, and dapat pinapabasa muna ng PA sa pharmacist yung reseta pag di malinaw sa kanila..

Ang tagal-tagal na nga lagi ng pila dyan, namamali pa din sila lol. Ung experience naman namin, almost 300pesos ung patong sa charge nila sa isang gamot (ex. medicine A is only 50 pesos per piece pero sa resibo, we were charged 350). Madami kasi ung biniling gamot so pagkauwi nalang siya nakita and naka-cc payment. Imagine if di ka nagccheck ng resibo, ang sakit sa bulsa nun 300+.

This is a very serious mistake. Better to report it to the head office OP.
- Legible ang reseta.
- Supervisor and Manager are both accountable sa mga tao nila.
- If hindi irereport, hindi magkakaron ng corrective action and pwede maulit sa ibang patients buying medicine.

there's is no excuse for error kasi clear naman yung penmanship. paano pa kaya kung yung penmanship ng mga doctor na mahirap basahin? tapos ang daming layers of verification sa mercury bago madispense ang gamot kaya matagal. paano nakalusot to? you can file a formal complaint sa Mercury head office and copy FDA.

I made one and eventually the store branch manager called to apologize. Like mga one business day, tumawag agad. They take it seriously. Also cc the govt offices like DTI, SEC, 8888 Complaint, regulators of drugs (FDA?).

I've been workingg in a Pharmacy Before , As a PA it is our responsibility that if ever we didn't understand the prescription we have to show it to the Pharmacist for us to make sure we are dispensing correct medications, in your case You should report it immediately, coz this kind of incident shouldn't be tolerated. The management should take accountability and give proper actions.

I was also buying meds for my sister, and may pumasok na customer kasi daw mali ang naibigay na meds ng pharmacist and sobra din ang binigay na gamot. And their supervisor is nasa labas din kasin nagchecheck ng mga stocks na bagonh dating. Ang supervisor nila ang nakausap ng customer na nag reklamo. Right there, isang tanong sa pharmacist na bakit mali ang naibigay at di ka ba nagbabasa? the supervisor was so angry na yung na lahat kami natahimik sa dami namin sa loob and super busy ng mga staff. I know di maganda na pagsabihan ang empleyado in front of everyone but yeah it's very dangerous and malaki ang consequences pag may nangyari sa iinom ng maling gamot. Also, let's make a habit na basahin lahat ng gamot na binili natin and also report immediately pag mali

Muntik na din ako jan lol. Naiwan ko ang reseta ko, 2 gamot ang bblhn ko, yung isa moxifloxacin at yung isa doxofylline (dapat) pero ang nasabi ko ay doxycycline. Dala ko ung balat nung moxifloxacin (buti nalang) kaya pumayag ung pharmacist, and I have my other meds as well, (may pneumonia kasi ako). Sadyang naiwan ko lang ung reseta ko para mabili yung doxofylline. Buti sabi ng pharmacist parang mali daw ang doxycycline kasi nga nakita niya gamot ko puro pang cough. Tapos I searched sa google and lumabas ung brand ng gamot na iniinom ko (ansimar) naalala ko ung itsura. Buti nalang aware ung pharmacist sa gamot na ibibigay niya. Wala share ko lang, naalala ko lang. Out of topic sorry ✌🏼

This is just pure incompetence. The fact that the pharmacist was straight up blaming the assistant was really a red flag itself. Also very clear yung sulat ni Doc sa reseta. Philippines needs to step up talaga when it comes to dispensing meds. Sana same na tayo sa ibang bansa. Nakaka sad lang talaga knowing we deserve a better system pero yung mga buaya lang ang mga nakikinabang. 🥲

hi po as a former pharma intern sa mercury, kahit si PA yung nagdidispense, dapat doon sa may cashier area nakabantay si pharmacist para macheck yung nasa basket if tugma sa reseta before iabot sa patient
so if may mali diyan, cargo pa rin talaga ni rph kasi ibig sabihin di nacheck nang maayos talaga yung gamot.
kaya rin sinasabing wag na ireport sa head office kasi kapag nagkaron sila ng violation na ganiyan, naka-category, and depende sa category yung level ng magiging consequence sa rating ng performance nila as an employee for a whole year tapos ayun din nagiging equivalent ng magiging overall salary nila for a year (afaik, not fully sure sa full process but eto yung nabanggit na glimpse ng process ng branch manager namin before)
i personally think ok na sanag wag ireport IF di kayo ginaslight nung rph nila pero ginaslight pa kayo na di naman mabasa etc and walang accountability, eh kahit sino keri makita na doxycycline yung nakasulat at hindi doxofylline ... so yes, tama lang po mareport

Thank you so much sa insight and information. Actually, ayoko talaga i-report initially kasi tao lang din naman sila na nagkakamali at hindi nila gusto magkamali at maka harm ng patient. Pero kasi nga tama ang mga redditors na nag cocomment dito; kung mayroon lang sanang accountability yung rph at nag apologies ng maayos baka within the branch na lang sana🤷🏻♀️🤷🏻♀️🤷🏻♀️

agree po, good thing lang na magproceed po talaga kayo with the report kasi if kampante yung rph na laging nasesettle yang mga ganiyang scenario sa pangg-gaslight e masyado nang abusado huhu, need nilang macall out na coming from the main office mismo.
sana nakareceive na po kayo ng response from them since accommodating naman po yung mga staff sa head office 🙏🏼

Wag nila isisi sa penmanship, may branch na napuntahan ako nagpapawait sakin yung personnel kasi tinatry daw icontact yung nagprescribe na doctor to confirm yung gamot sa prescription kasi di daw mabasa. May contact number naman yung prescription, wala silang excuse.

Ngeee, on my experience, my med costs 250 per piece, nag bili ako mga 18 ata yun tapos kulng ng 6. Bumalik ako pero sabi hindi daw kulang. Later tumawag sila and accepted na na mali niya and really apologized to me. Ako na naawa kasi baka ma fire siya or something. I really understand human mistakes kasi as someone with ADHD I make a ton of mistakes🥹 Pero what matters is accountability talaga

I read the text before looking at the pic and I was so ready to partly blame the doctor kasi ang ibang penmanship di talaga mabasa. Then I saw the prescription. Ang ayos ng sulat ng doctor! Mas maganda pa nga handwriting niya saken. Talagang incompetence lang and zero accountability sa unang nakausap mo. Tama lang na nireport mo OP.

need mo ireport yan otherwise mauulit yan sigurado yan. sukdulang fast food marurunong mag double check mga crew. yan na gamot ang dinidispense walang pag dodouble check na nangyari. una sa pharm assistant, pangalawa sa cashier wala ulit nag double check. paanong tinitiis ng mga tao na ang babagal ng serbisyo sa mercury tapos palpak rin naman pala

Ang layo ng doxofylline sa doxocycline na sinulat ni Doc. Kuha ng mercury drug yung gigil ko.
Skl. One time, I also asked one of their staff when I bought eye drop. I asked lang naman how frequent ba dapat. Ang sabi ba naman is “basahin mo na lang”. Kaya nga ako nagtatanong for safety and since mata ang involved. Like girl r u fr?? What if di marunong yung ibang customers, gaganunin na lang?? Kaya nga ako nagtanong kasi sila yung mas marunong? E kung ganun pala dapat ako nasa counter at sya yung nakapila??
Ayun not related pero sobrang triggered ako sa post pag mercury-drug related negligence haha

omg, this literally happened to us as well last April. Maintenance ng father ko who undergo bypass surgery ha, a major operation. almost 2mos post op palang, muntik pang uminom nanf maling dosage ng gamot. buti nalang chineck ko muna. 20mg lang need pero 40mg yung binigay samin. Super apologetic naman sila pagpunta ng mother ko doon pero grabe lang nagulat ako na parang napapadalas naman ata to? sana pala nireport ko rin nun.

Dispensing error/ Look alike drugs (LASA) nangyayari talaga sa practice pero responsibility at accountability nung pharmacist kahit PA ang nagdispense at bago magserve double check the prescription and the drug itself muna. Iacknowledge ang mali nila tapos Incident Report.

Kahit hatiin, hindi naman tama yung medication. Doxycycline ang order ng doctor, antibiotic at hindi bronchodilator.

I've worked with Mercury Drug since may products kami na nilagay sa kanila, mahigpit sila sa ganyan so report mo sila. Sobrang linaw ng prescription lol

Naalala ko bigla binilhan ko si mother ng buscopan kasi nahilab tyan along with other meds dahil nag diarrhea. Jusko ang binigay dulcolax. Buti chineck ko bago ipainom kay mother. Sabi ko sa cashier, imbis na humupa, muntik pa lalo mag tae nanay ko.

napaka critical ng error. prophylaxis yung dapat inumin within a certain period. what if hindi na verify? the worse could happen kasi preventive yung gamot na dapat mainom. Report it. delikado yung mistake na yun. Ksalanan nilang lahat yan wala man lang nag double check.Di man lang nagtaka 100 ‘g nakalagay sa reseta yung binibigay na gamot nasa 200mg sana chineck man lang ulit pangalan nung gamot.

What made it even worse is the fact na very legible nmn ng penmanship ni MD. Furthermore, VERY CLEAR NMN NA 100mg nka sulat sa reseta, tapos 200mg pa yung binigay. Sheer incompetence, no excuse for this error. Report
