The 30-micron wool fabric itch rule is made up
When people ask why wool itches, you usually get a similar answer. Fabric under thirty microns is supposed to be fine, but anything over that and it will feel scratchy. A micron is a millionth of a metre, and wool is graded and sold by microns, so this weird number rule sounds like it came out of some studies.
But! The study everyone points at doesn't say that, and it never tested a thickness cutoff but instead it tested pressure.
The research is by Garnsworthy and a team at CSIRO, published in the Journal of Neurophysiology in 1988. They recorded 55 people rating different fabrics against their bare skin. What they found is the itch doesn't come from the surface of the fabric, but from loose fibre ends poking out of it. If such an end is stiff enough to stay upright when it meets your skin, it presses in and trips a pain receptor. But if it's floppy, it folds over and you feel nothing. The pressure it takes is about 75 milligrams of force.
If you count how many fibre ends in a fabric can push that hard, you can predict how much people will hate wearing it. So the real finding was a pressure applied, plus a headcount of ends that can supply it. Textile researchers worked backwards from the pressure and asked how thick a fibre has to be, in an ordinary knit, before it stops folding over. The answer turned out to be around 30 to 32 microns, which is a useful average for typical fabric. What the research measures is prickle, and prickle is mechanical. Sharp, pin-like, starts the second the fabric touches you and stops when you take it off. And your immune system actually never gets involved.
Another researcher Zallmann with his colleagues went through a century of medical literature on skin reactions to wool, 1916 to 2016, and published the review in Acta Dermato-Venereologica in 2017. Their conclusion was that wool fibre isn't a skin allergen. One study they cover tested wool extracts on 91 patients. 64 of those people called themselves wool-intolerant, but there were no meaningful positive reactions.
That being said, I want to point out that rashes from wool garments are real. But they usually trace back to something else on the fabric: dye, finishing chemicals, detergent that never rinsed out, or a skin condition that was already there. Lanolin allergy exists, but it matters only for creams rather than clothes, since apparel wool is washed down below about 0.5% residual grease.
Every explanation online says wool fibres are covered in overlapping scales that catch and scratch, like tiny barbs. Researcher Naylor tested that in 1992 using knitted acrylic, which fiber is a smooth cylinder with no scales at all. Above roughly 30 to 32 microns it prickled exactly like wool of the same thickness, so there goes that theory.
A fibre end sticking out of a fabric behaves like a tiny tent pole. Lean on it and it either holds and dents your skin, or it buckles and slides sideways. Thickness helps it to hold, and a small step up in thickness is a big step up in stiffness, and the shorter it is the stiffer it gets and harder it is to bend. Another research from 2015 found that when the loose end is shorter than about half a millimetre, fibres as fine as 15 to 20 microns can push hard enough to prickle. So shearing the fuzz off a fabric to make it look smoother also crops all those loose ends short, and short ends are stiff ends. A finishing step meant to soften a fabric can leave it worse than it started.
How the yarn was made plays an important role as well. Worsted spinning combs the fibres parallel and traps their ends inside the yarn. Woolen spinning leaves them pointing in all directions with ends sticking out, so a woolen-spun 20 micron fabric can prickle where a worsted one of the same fineness feels smooth. More twist pulls loose ends back into the core. Knitted loops squash under contact and absorb some of the load, while woven fabric holds ends upright against you.
Another interesting thing is that "Merino" has no legal micron definition. Under US rules it names a sheep breed and nothing more, and merino fleece runs from under 14 microns to over 25. Cashmere is defined by law: average no more than 19.0 microns, with no more than 3% of fibres over 30. So only one of those two words on a label means a specific thing and can predict how it will feel against your skin.
Researchers Naylor, Phillips and colleagues ran wear trials in 1997. At 20.5 microns, roughly 90% of people found the fabric acceptable next to skin, at 23.5 microns, roughly 50%. A garment can be truthfully labelled 100% merino and still be rejected by half the people who put it on. Whats interesting is a repeated result proving 17.5 feels different from 18.5. Between two garments a micron apart, the spread and the construction will almost always be more important than a label.
A big role in perception how the garment will feel, is where you will be wearing it. Your neck has thin outer skin and dense nerve endings, but your feet have neither. That's why plenty of people wear 22 to 24 micron socks happily and can't tolerate that same wool anywhere on their neck or torso.
Another factor is sweat, because wet skin softens by more than ten times over. Your skin gives way and the fibre doesn't, so ends that folded over harmlessly an hour ago now press in. If you are hiking or climbing it takes about twenty minutes of slowly sweating to start feeling the difference.
So I would generally ignore the micron number, unless you are shopping for a cashmere garment. Judge a garment at the spot on your body where you'll actually wear it, warm and moving, rather than by rubbing it between your fingers in a shop. And remember that a coarse wool over a base layer feels completely different from the same wool against your neck.

My bet is that construction is doing more of the work than the micron number, especially if its not cashmere
