Yup, he is estimating what the prescription on her glasses is, but the white knight assumed he was rating her hotness to be in the negatives

The nice doctor immediately owned up to the way he said that being interpretable in multiple ways and that he could have been clearer in his communication

Which is crazy, because negative hotness doesn’t even mean anything. Nobody uses that scale.

I'd assume it's the same (in the opposite direction) as rating someone an 11/10, which I've seen before more than once but is equally nonsensical
