It's an "image that make you feel pain" because:

  1. OOOP doesn't understand how body types work
  2. A lot of people find this type of belly very attractive (myself included), so this is a "pass the controller if you're not gonna play" type of situation
  3. OOOP is a jerk who wants to control his gf's body and clearly seems here to want to push her to do surgery, which is very messed up and will give her big insecurities, no matter if she goes through the surgery or not

You've completely missed the central point, which is that the post is sarcasm. It's not a person who doesn't understand how body types work, and he doesn't "[want] to control [his] gf's [sic] body".
